Regulation of Compound Astragalus and Radix Polygoni Multiflori Mixture on Renal Expression of Podocin and Nephrin in Metabolism Syndrome Rats

摘  要:目的:观察复方黄芪首乌合剂对代谢综合征大鼠早期足细胞损伤的保护作用及其机制。方法:采用高糖高脂饲料喂养法建立实验性代谢综合征大鼠模型,以复方黄芪首乌合剂(高、中、低剂量组)灌胃治疗,8周末收集尿标本检测24 h尿蛋白(24 hPro)、尿微量白蛋白(MA)和尿白蛋白肌酐比值(albumin to creatinine ratio,ACR);免疫组织化学法检测肾皮质Nephrin和Podocin的蛋白表达;逆转录-聚合酶链反应(RT-PCR)检测肾皮质Nephrin和Podocin mRNA的表达;电镜下观察肾脏基底膜和足细胞的变化。结果:造模大鼠24 hPro、MA和ACR较正常对照组明显增加(P<0.01);肾小球基底膜显著增厚,足细胞与对照组大鼠比较明显减少,足突排列紊乱,足突融合;肾皮质Podocin、Nephrin蛋白和mRNA表达明显低于正常对照组(P<0.05)。经复方黄芪首乌合剂治疗8周后,24 hPro、MA、ACR均明显减少(P<0.05);肾小球基底膜病变减轻,细胞外基质增多减轻,足细胞数目增多,足细胞融合减轻;肾皮质Podocin、Nephrin蛋白和mRNA表达分别较模型组显著增加(P<0.05)。结论:复方黄芪首乌合剂对代谢综合征大鼠肾脏足细胞损伤有保护作用,其作用机制可能与其参与调节代谢综合征大鼠肾组织Nephrin和Podocin的蛋白和mRNA表达有关。Objective : To investigate the effects of compound Astragalus and Radix polygoni muhiflori mixture on renal expression of podocin and nephrin in metabolism syndrome (MS) rats. Methods: The experimental MS rat model was - established through a high sugar high fat diet. Animals were administered with compound Astragalus and Radix polygoni multiflori mixture. After 8 weeks,24 h urine samples were collected to detect 24 hPro, MA and ACR. The renal cortical protein expressions of nephrin and podocin were detected with immunohistochemistry. The renal cortical gene expressions of nephrin and podocin were detected with RT - PCR. The changes of glomerular basement membrane (GBM) and podo- cyte were observed with electron microscopy. Results : The levels of 24 hPro, MA and ACR of model rats were increased significantly compared with those of normal rats ( P 〈 0.01 ). The width of GBM was increased significantly, and podo- cyte were fused. The number of podocyte was decreased significantly compared with that of normal rats. The expression of renal cortical gene and protein of nephrin and podocin were decreased significantly compared with those cif normal rats. After treated with compound Astragalus Polygonum polygoni multiflori mixture, the levels of 24 hPro, MA ~and ACR we- re decreased significantly compared with those of tnodel group ( P 〈 0.05 ). The expressions t)f renal cortical gene and protein of nephrin and podocin were increased significantly compared with those of model rats ( P 〈 0.05 ). Conclusion : Compound Astragalus Polygonum polygoni multiflori mixture could prevent the renal podocytes of MS rats, and it maybe depends on the participation of the renal cortical gene expression of nephrin and podocin.
