早期糖尿病肾病血清HGF,Cys C和TGF-β1水平及平肾汤干预的影响

摘　　要：探讨早期2型糖尿病肾病(diabetic nephropathy DN)血清肝细胞生长因子(hepatocyte growth factor,HGF)和血清胱抑素C(cystatin C)、转化生长因子-β1(transforming growth factor-β1,TGF-β1)水平及中药平肾汤干预的影响。选择69例2型DN患者,随机分为观察组36例与对照组33例,观察组用中药平肾汤每天1剂,分2次服用。对照组用肾炎舒片每次6片,1日3次口服。8周为一疗程。治疗前后应用酶联法和免疫比浊法进行HGF,Cys C和TGF-β1水平测定,并与30名正常健康组作比较。研究表明治疗前2组患者HGF水平明显低于健康组(P<0.01);而Cys C,TGF-β1明显高于健康组(P<0.01)。治疗后2组患者HGF水平均提高,但观察组明显高于对照组(P<0.01),Cys C,TGF-β1明显低于对照组(P<0.01),血清HGF水平与Cys C,TGF-β1水平呈负相关;观察组治疗后尿白蛋白排泄率(urinary albumin excretion rates UAER),尿β2-微球蛋白(β2-MG),24 h尿蛋白定量比治疗前明显降低(P<0.01),且治疗后上述指标明显低于对照组(P<0.01)。结果提示血清HGF和Cys C,TGF-β1水平的测定对早期DN的发生和发展有重要的临床意义。中药平肾汤能有效干预DN血清HGF和Cys C,TGF-β1和尿液指标水平。Study the serum level of HGF,Cys C and TGF-β1 in type 2 diabetic nephropathy （DN）, the infect of Pingshen decoction on those index. Selected 69 cases of 2 type DN and randomly devided into therapy group （36cases） and control group （33 cases）.The therapy group were treated with Pingshen decoction 1 dose/d, bid po.The control group were treated with NephritisShu tablet, 6 tablet, tid po. 8 weeks was a course. Before and afeter treatment,we examine the serum level of HGF,Cys C and TGF-β1 by ELISA and immunonephelometry, and compare with 30 cases of healthy control group. The study demonstrates that before treatment, the serum level of HGF in both groups were significantly lower than healthy control group （P〈0.01）, but Cys C,TGF-β1 were significantly higher （P〈0.01）. After treatment, the serum level of HGF of both groups were increased. The serum level of HGF of therapy group were significantly higher than of control group（P〈0.01）, but the serum level of Cys C and TGF-β1 were significantly lower than control group（P〈0.01）. The serum level of HGF was correlated negatively with Cys C,TGF-β1. In control group, the UAER, urine β2-MG and quantity of 24-hour urine protein were significantly decreased after treatment （P〈0.01）. The index of urine of therapy group were significantly lower than control group（P〈0.01）. Results indicate that test of serum level of HGF and Cys C,TGF-β1 of diabetic nephropathy have important clinical significance. Pingshen decoction can effectively intervene in the serum level of HGF and Cys C,TGF-β1 and index of urine.
