摘  要:目的 探讨中国人群β3肾上腺素能受体(β3-AR)基因Trp64Arg突变与糖尿病足的关系.方法 应用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)方法检测110例2型糖尿病患者(T2DM组)[其中糖尿病足患者58例(DFU组),单纯2型糖尿病患者52例(N-DFU组)]和42例正常人(正常对照组)β3-AR基因的Trp64Arg突变,比较各组的基因型(TT型,TA型或AA型)和基因频率(T或A);并进行人体测量学和代谢指标的检测.结果 (1)β3-AR基因Trp64Arg基因型频率符合Hardy-Weinberg平衡定律,T2DM组β-AR基因64位Arg突变患者,其体重指数和稳态模型评估-胰岛素抵抗指数(HOMA-IR)显著高于无突变组(P=0.007 2,P=0.038 6).(2)在T2DM组,Arg64携带者血高密度脂蛋白-胆固醇水平明显低于非携带者(P =0.047 5).(3)DFU组Arg64等位基因频率明显高于N-DFU组(x2=5.147,P<0.05),而T2DM组与正常对照组β3-AR基因型及等位基因频率无统计学意义(x2=0.067,P >0.05).结论 β3-AR基因多态性与糖尿病足存在相关性,Ar64等位基因可能是糖尿病足的危险因素之一.Objective To explore the relationship between Trp64Arg polymorphism of the β3-adrenergic receptor(β3-AR) gene and diabetic foot in Chinese subjects.Methods The mutational genotype of Trp64Arg in β3-AR gene was measured by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) in 110 patients with type 2 diabetes mellitus (T2DM group) [(58 with diabetic foot ulcer (DFU group) and 52 without (N-DFU group)] and 42 normal healthy subjects (normal control group).The genotype (TT,TA or AA) and allele frequencies(T or A) were compared among groups.The anthro-pometry and metabolic indices were also compared between groups.Results (1) No significant deviation from Hardy-Weinberg equilibrium was found in the β3-AR Trp64Arg genotype frequency.In T2DM group,body mass index and homeostasis model assessment for insulin resistance index (HOMA-IR) in Arg allele were significantly higher than those without mutations allele (P =0.007 2,P=0.038 6).(2) In T2DM group,Arg64 carriers had lower level of high density lipoprotein-cholesterol than those without this mutation (P=0.047 5).(3)The frequency of Arg64 allele in DFU group was greatly higher than that of N-DFU group(x2=5.147,P〈0.05),while no difference in β3-AR gene genotype and allele frequency was observed between T2DM group and normal control group(x2=0.067,P 〉0.05).Conclusion β3-AR gene polymorphism is associated with diabetic foot.The Arg64 allele may be one of the risk factors of diabetic foot.
