子宫输卵管碘油造影术并发淋巴、静脉逆流的影像学分析

摘　　要：目的：分析不孕症患者在进行子宫输卵管碘油造影时并发淋巴、静脉逆流的发生机制及影像学表现，从而达到预防和减少逆流的发生，尽可能避免并发肺栓塞、脑梗死等严重后果。方法：均使用荷兰飞利浦公司的PHILIPS 1000MA数字胃肠机完成，采用40%碘化油造影剂5~20 ml，分析子宫输卵管碘油造影时并发淋巴、静脉逆流12例患者X线影像学表现。结果：造影剂淋巴、静脉逆流是由于子宫畸形、内膜炎症、输卵管梗阻、机械性内膜损伤、药量过多、注药压力过大等多种因素单独或共同作用而产生的并发症。其中技术原因4例，子宫输卵管本身原因8例。结论：提高对子宫输卵管碘油造影时并发淋巴、静脉逆流发生机制的认识，在造影前做好充分准备，正确把握好女性生理周期最佳时间段，谨慎操作，术中做好电视实时密切观察，尽可能减少或避免发生逆流，防止肺栓塞、脑梗死等严重后果。Objective：Analysis on the mechanism and imaging findings of hysterosalpingography concurrent lymphatic and venous reflux of sterility patients,so as to prevent and reduce the occurrence of countercurrent,and to avoid pulmonary embolism,cerebral infarction and other serious consequences as far as possible.Method：Used the Netherlands,1000 PHILIPS MA digital gastrointestinal tract to completed,with 40 Percent iodized oil 5-20 ml.Analysised on the mechanism and imaging findings of hysterosalpingography concurrent lymphatic and venous reflux of 12 cases.Result：Contrast agents lymphatic and venous reflux was a complication which was caused by uterine malformations,endometrium inflammation,tubal obstruction,mechanical lining damage,overdose, injection pressure,and other factors alone or together.The technology cause 4 cases,uterine tubal itself cause 8 cases.Conclusion：Improve the understanding of uterus oviduct iodine oil imaging mechanism of concurrent lymphatic and venous reflux,fully prepared before the procedure,correctly grasp the best time of the female physiological cycles,proceed with caution and do good TV real-time observation,so as to prevent and reduce the occurrence of countercurrent,and to avoid pulmonary embolism,cerebral infarction and other serious consequences as far as possible.
