糖尿病患者腰围身高比值与2型糖尿病肾病的相关性

摘　　要：目的探讨糖尿病患者腰围身高比值(WHtR)与2型糖尿病肾病的相关性。方法选择426例2型糖尿病患者,根据有无并发糖尿病肾病分为糖尿病肾病组(DN)103例和非糖尿病肾病组(NDN)323例。比较两组2型糖尿病患者的血压、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、糖化血红蛋白及WHtR。logistic回归分析WHtR与糖尿病肾病的相关性。结果 DN组患者病程、收缩压、TG、WHtR分别为(8.7±7.6)年、(140±20)mmHg、(2.59±2.11)mmol/L、0.58±0.06,较NDN组[分别为(6.6±6.2)年、(131±17)mmHg、(2.01±1.69)mmol/L、0.55±0.06]明显增高,差异均有统计学意义(P<0.05,P<0.01),调整了性别、年龄、糖尿病病程、血压、血脂谱、糖化血红蛋白后,WHtR与糖尿病肾病明显正相关(OR=594.899,95%CI:8.731~40 534.869)。结论 WHtR是2型糖尿病肾病的独立危险因素,积极防治肥胖可能有助于糖尿病肾病的防治。Objective To explore the correlation between the waist-to-height ratio （WHtR） and diabetic nephropathy in patients with type 2 diabetes mellitus （T2DM）. Methods 426 patients with T2DM were divide into diabetic nephropathy （DN） group （103 eases） and non diabetic nephropathy （NDN） group （323 eases）. Blood pressure, blood lipids, HbA^c levels and WHtR were compared between two groups. Logistic regression was used to analysis the correlation between WHtR and diabetic nephropathy. Results As compared with NDN group [（6.6±6.2） years, （131±17） mmHg, （2.01±1.69） retool/L, 0.55±0.06, respectively], the diabetic duration [（8.7±7.6） years], systolic blood pressure （SBP） [（140±20） mmHg], triglyeeride （TG） [（2.59±2.11 ） mmol/L] and WHtR （0.58±0.06） in DN group increased significantly （P〈0.05, P〈0.01 ）. After adjusting the sex, age, duration, blood pressure, blood lipids and HbAlc, there was a positive correlation between WHtR and diabetic nephropathy （OR=594.899,95%CI： 8.731- 40 534.869）. Conclusion WHtR is an independent risk factor of diabetic nephropathy, the prevention and treatment of obesity may be useful for preventing the diabetic nephropathy.
