24例巨大裂孔性视网膜脱离的手术治疗观察

摘　　要：目的观察巨大裂孔性视网膜脱离的手术治疗效果。方法根据裂孔范围及视网膜玻璃体增殖程度不同,采用不同手术方法,巨大裂孔性视网膜脱离患者24例（24眼）,对14例裂孔范围90°~180°者行常规巩膜外冷凝,环扎及硅胶外垫压术。其余10例裂孔范围〉180°~270°、视网膜玻璃体增殖严重者行标准三通道玻璃体切除术,采用气液交换平复视网膜,术后8眼眼内填充硅油,2眼眼内填充C3F8气体。结果随访3~12个月,视网膜最终复位21/24眼,复位率为87.58%。结论采取合适的手术方法,可以有效提高巨大裂孔性视网膜脱离的治愈率。[ Objective ] To observe effect of surgical treatment for giant retinal detachment. [ Methods ] According to the different in scope and extent of retinal vitreous proliferation, using different operation methods, giant retinal detachment patient 24 cases （24 eyes）, 14 cases of 90-180 degrees in scope were treated with scleral buckling, cerclage, cryotherapy and laser photocoagulation; The rest 10 cases with retinal detachment underwent vitrectomy and retinal reattachment; 8 cases used silicon oil as tamponade and 2 cases used gases （C3F8）. [ Resluts ] During 3-12 month follow-up,eventually reset retinal eye, 21/24 reset rate is 87.58%. [ Conclusion ] Use appropriate surgical method, can effectively improve the cure rate of giant rhegmatogenous retinal detachment of retina.
