摘  要:目的评价丙氨酰谷氨酰胺强化的肠外营养对重型颅脑损伤患者肠黏膜屏障的保护作用。方法 100例重型颅脑损伤患者随机分为肠外营养组和丙氨酰丙氨酰谷氨酰胺组,每组50组,2组在治疗后第1、5、9天分别测定血浆二胺氧化酶含量、D乳酸含量、血浆内毒素水平。结果与肠外营养组相比,丙氨酰谷氨酰胺组第1天血浆二胺氧化酶含量、D乳酸含量、血浆内毒素水平差异无统计学意义(P>0.05),第5、9天血浆二胺氧化酶含量、D乳酸含量、血浆内毒素降低,差异有统计学意义(P<0.05)。结论重型颅脑损伤患者在使用含有丙氨酰谷氨酰胺的肠外营养时,肠黏膜通透性减低,肠黏膜屏障得到了保护。Objective To investigate the impact of alanyl glutamine enriched parenteral nutrition on plasma diamineoxidase, D-lactate acidosis and endotoxin level after severe brain injury. Methods One hundred patients undergoing severe head injury were delivered into parentenal nutrition group and alanyl glutamine group at random. They were supported with parentenal nutrition group and alanyl glutamine group from 1 -9 days after trauma. Plasma diamineoxidase, D-lactate acidosis and endotoxin level were determined at the 1,5,9 day after trauma respectively. Results Compared with parentenal nutrition group, alanyl glutamine group could downregulate the level of plasma diamineoxidase, D-lactate acidosis, endotoxin level at the 5,9 day after treatment. Conclusion The glu-enriched parenteral nutrition could protect the gut barrier, attenuate intestinal permeability.
