摘 要:目的:探讨中国常武地区汉族人群妊娠相关血浆蛋白A(PAPP-A)基因IVS6+95多态性与急性心肌梗死(AMI)易感性的关系。方法:AMI组308例,对照组(无冠状动脉狭窄的非冠心病者)213例,采用聚合酶链反应一限制性片段长度多态性方法分析PAPP-A基因IVS6+95多态性。结果:AMI组和对照组PAPP—A基因IVS6+95位点均检出GG、CG和CC3种基因型。单因素和多因素分析均未发现CG和CC基因型频率分布在两组之间有统计学差异。与对照组相比,AMI组的GG基因型(52.58%:43.51%,P=0.049)和G等位基因(73.71%:67.05%,P=0.024)频率分布均较低;经多因素Logistic回归分析显示,AMI组的GG基因型(OR=0.642,95%CI:0.428-0.962,P=0.032)和G等位基因(OR=0.677,95%CI:0.487~0.942,P=0.020)频率仍低于对照组。结论:在中国常武地区汉族人群中,AMI患者和无冠状动脉狭窄的非冠心病者均存在PAPP—A基因IVS6+95多态性,且后者与AMI的易感性降低密切相关,提示GG基因型和G等位基因可能是减少AMI发病风险的遗传因素之一。Objective:To investigate the association of pregnancy-associated plasma protein-A (PAPP-A) gene IVS6 + 95 polymorphism with susceptibility of acute myocardial infarction (AMI) in the Chinese Han population of Changwu region. Method: The 308 patients with AMI and 213 control subjects who were not coronary heart disease (CAD) without coronary artery stenosis by coronary angiography were recruited into the study. The PAPP-A gene IVS6 + 95 polymorphism was measured by polymerase chain reaction and restriction fragment length polymor- phism. Result..GG, CG and CC genotype were detected in PAPP-A gene IVS6+95 loci in AMI group and control group. Single factor and multi-factor analysis did not find that CG and CC genotype frequency distribution difference existed statistically between two groups. The frequencies of GG genotype (43.51% vs 52.58%, P=0. 049) and G allele (67.05% vs 73.71%, P=0. 024) in AMI group were lower than those in control group; Multiple logistic regression analysis showed that the frequencies of GG genotype (OR=0. 642, 95%CI:0. 428-0. 962, P=0. 032) and G allele (OR=0. 677, 95%CI: 0. 487-0. 942, P=0. 020) were still lower in AMI group than those in control group. Conclusion:The PAPP-A gene IVS6 + 95 polymorphism exist in patients with AMI and non CAD subjects without coronary artery stenosis, and is closely related to reduce susceptibility of AMI, suggesting GG genotype and G allele may be one of genetic factor to reduced risk of AMI in the Chinese Han population of Changwu region.
