创伤性髌上囊积脂血症的临床与MRI诊断分析

摘　　要：目的探讨创伤性髌上囊积脂血症的临床及MRI表现特点。方法回顾性分析20例创伤性髌上囊积脂血症患者的临床及0.35T MRI资料。结果 MRI表现为髌上囊内脂-液平及脂-液-液平,上层均为脂肪信号,MRI显示以轴位及矢状位为佳,MRI还较好显示了伴发的膝关节其他创伤及膝关节退行性骨关节病。临床上经过冷敷、穿刺抽液、理疗、复发及顽固性者经负压吸引治疗后均治愈。结论髌上囊创伤性积脂血症具有较为特异性的MRI表现。Objective To investigate the clinical and MRI features of traumatic lipohemarthrosis in suprapatellar bursa. Methods Clinical and 0.35T MRI data of 20 persons with traumatic lipohemarthrosis in suprapatellar bursa was retrospectively analyzed. Results MRI showed suprapatellar bursa fat - fluid level and lipid - fluid - fluid level, the uppermost level was undoubtedly lipid signal. The axis and sagittal MR imagings showed better. MRI also showed other injuries and degenerative osteoarthritis of the knee. All were cured after the therapy of cold compress, puncture, physiotherapy, the recurrent and refractory were cured by negative pressure suction. Conclusion Traumatic lipohemarthrosis of uprapatellar bursa has typical MRI appearances.
