摘　　要：目的调查2007—2011年四川省阿坝州儿童大骨节病病情，正确评估综合防治措施效果，为探索有效防控措施提供科学依据。方法结合《阿坝州扶贫开发和综合防治大骨节病试点工作总体规划》的实施，2007—2010年在全州大骨节病普查基础上，按照病情轻、中、重程度分层设立58个病情监测点，每年对点内6～13岁儿童进行大骨节病临床检查和拍摄右手正位X线片，观察病情动态变化。结果5年共临床检查6—13岁儿童20054人，2007和2008年共查出I度及I度以上大骨节病患者18人，2009—2011年，临床未检出I度及I度以上大骨节患者，临床检出率从0．27％（12／4463）下降到0。5年共拍摄6—13岁儿童手部X线片14189张次，X线阳性片180张，X线阳性检出率从2007年的3．43％（33／962）下降到2011年的0．56％（24／4273）。结论通过4年的综合防治，四川省阿坝州儿童大骨节病新发得到有效遏制，综合防治措施取得明显效果。Objective To evaluate the status of Kaschin-Beck disease （KBD） in ABa Prefecture Sichuan Province, and to properly assess the effects of comprehensive prevention and control measures, and to provide a scientific basis for exploring effective prevention and control measure. Methods According to the ＂Master Plan for Aba Poverty Alleviation and Development and Pilot Study of Integrated Control of KBD＂ and based on the 2007 statewide survey on KBD, 58 KBD survey sites were stratified based on the disease situation of mild; moderate and severe state to survey the dynamic change of KBD. Children aged 6 - 13 with KBD were examined clinically and right hand anteroposterior X-ray was taken each year. Results A total of 20 054 children aged 6 to 13 were clinically examined in five years, 18 were identified as degree I or higher patients with KBD from 2007 to 2008. No patient was found between 2009 and 2011, and the clinical detection rate declined from 0.27%（12/4 463）to zero. A total of 14 189 right hand anteroposterior X-ray films were checked, and positive X-ray films were 180; X-ray positive rate declined from 3.43% （33/962） in 2007 to 0.56% （24/4 273） in 2011. Conclusion After four years of comprehensive prevention, children KBD has been effectively contr611ed, and the comprehensive prevention and control measures have achieved remarkable results.
