摘 要:[目的]通过观察关节腔注射双醋瑞因(Diacerein,DAR)对SD大鼠骨关节炎(Osteoarthritis,OA)形态及关节滑膜组织单克隆抗体(ED1)、软骨Ⅱ型胶原蛋白(COL2)表达的影响,探索关节腔注射双醋瑞因对SD大鼠骨关节炎的治疗作用及机理。[方法]35只SD大鼠,15只为正常组,其余20只手术剪断双侧前交叉韧带,术后第3 d起关节腔内注射治疗,左膝注射二甲基亚砜(对照组)0.1 ml,右膝注射DAR液(治疗组)0.1 ml。于注射后3 d,1、2、4、8周分5次分别处死3只正常组大鼠和术后治疗SD大鼠4只,取左右全膝关节;对比术后大鼠左右膝关节软骨形态及滑膜组织ED1和软骨组织COL2的变化,并与正常组比较。[结果]二甲基亚砜组(对照组)手术4周后肉眼和镜下观察可见软骨形态明显改变,8周时软骨破坏最为明显,DAR治疗组(治疗组)软骨破坏不明显;二甲基亚砜对照组(左膝)在3 d,2、4周时滑膜均可见较多ED1阳性细胞数,先升高后逐渐下降,其后保持在一定水平,DAR治疗组(右膝)ED1阳性细胞数在各期均低于二甲基亚砜组(左膝),左右膝ED1阳性细胞有显著性差异(P<0.01),右膝ED1数8周时接近正常水平;二甲基亚砜组(对照组)(左膝)COL2含量减少较明显,至8周时几乎完全流失;DAR治疗组(右膝)COL2含量减少不明显,8周时接近正常。[结论]关节腔注射DAR能够抑制滑膜ED1阳性炎症细胞数量和提高软骨细胞间质COL2的含量,从而控制骨关节炎的炎性反应、抑制软骨变性及破坏,对骨关节炎的发展具有抑制、治疗作用。[ Objective] To investigate the therapeutic effect and mechanism of action of diacerein, injected intraarticularly on osteoarthritis in Sprague - Dawley (SD) rats, and observe the cartilage morphology and expression of EDI and COL2. [Method]Thirty -five healthy SD rats were divided into a normal group (n = 15) and a group of animals (n = 20) subjec- ted to an operation involving cutting of the left and right anterior cruciate ligaments. From the third day after the operation, in- traarticular injections of O. 1 mL dimethyl sulfoxide into the left knee ( control group) and 0.1 mL diacerein into the right knee (treatment group) were performed every day. On the third day,first week, second week ,fourth week, and eighth week after the injection, 15 normal rats were killed by five times and 20 SD rats injected with drugs were killed by five times. The left and right knee joints were extracted; the morphologies of the left and right knee cartilage were compared ; and the changes of synovial tis- sue ED1 and cartilage tissue COL2 were compared between the two groups. [ Result] At 28 days after the operation, an obvious change in the cartilage morphology of the control was noted by visual assessment and under microscopic observation. The de- struction of the cartilage was most obvious in the eighth week. However, the destruction of the cartilage in the diacerein treatment group was not obvious. In the third day, second week, and fourth week, many ED1 - positive synovial cells were observed in the dimethyl sulfoxide control group ( left knee), which first increased, decreased gradually, and then remained at a certain level. The ED1 -positive cells in the diacerein treatment group (right knee) decreased significantly compared with those in the dimethyl sulfoxide group (left knee) in each pe-riod; the number of cells in the eighth week was close to the normal level.
