MDCT对胃部异位胰腺的诊断价值

摘　　要：目的：探讨胃部异位胰腺的MDCT表现，以提高对本病的诊断准确率。方法：回顾性分析8例经手术病理证实的胃部异位胰腺的临床表现及MDCT特征。结果：8例胃部异位胰腺均为单发病灶，其中位于胃窦部2例，胃体部6例；8例均位于胃小弯侧。病灶长径1．8～2．7cm，短径0．9～1．4cm，长径／短径平均为1．9。病灶长轴均平行于胃壁。7例呈卵圆形，边界清晰，局部边缘模糊；1例呈不规则形，边缘模糊。8例均行平扫及增强检查，平扫8例均呈等密度，其中3例边缘显示脐凹征，1例中央见导管征。增强扫描，8例均呈明显持续强化，强化的趋势与正常胰腺一致。8例胃壁黏膜层均完整。结论：胃部异位胰腺为黏膜下病变，其MDCT表现具有一定的特征性，MDCT对该病的诊断与鉴别诊断具有一定的价值。Purpose： To investigate the MDCT imaging features of gastric ectopic pancreas, so as to improve its diagnostic accuracy. Methods： Clinical characteristics and MDCT imaging features of 8 cases with gastric ectopic pancreas proved by pathology were retrospectively reviewed. Results： All the 8 cases had single lesion. Among the lesions, 2 cases were located in the gastric antrum, the others were located in the gastric body. All lesions were located in the gastric lesser curvature. The long diameters （LD） of lesions ranged from 1.8 cm to 2.7 cm, the short diameters （SD） were from 0.9 cm to 1.4 cm, and the mean LD/SD ratio was 1.9. The long axes of lesions were all parallel to the stomach wall. Seven cases were ovoid with well-defined contours, but with part of blurred edges. Another case was irregular with blurred edge. All cases underwent CT non-enhanced and contrast-enhanced scans. On non-enhanced scan, all lesions showed homogeneous iso-density, three of which showed umbilical depression signs and one showed central duct sign. On contrast-enhanced scan, all cases showed bright and continuous enhancement. The trend of enhancement for lesions was in consistent with the normal pancreas. The overlying mucosa was clear and complete. Conclusion： Gastric ectopic pancreas is a kind of submucosal lesion, which has characteristic features on MDCT imaging. MDCT imaging plays an important role in the diagnosis and distinction between gastric ectopic pancreas and mesenchymal in stomach.
