特发性肉芽肿性乳腺炎的钼靶X线及MR表现与病理对比分析

摘　　要：目的 探讨特发性肉芽肿性乳腺炎（idiopathic granulomatous mastitis，IGM）的钼靶X线、MR图像特征及临床发病规律。方法 总结经手术病理证实的18例IGM的临床表现及钼靶X线、MR图像特征，其中18例均行钼靶X线检查，8例行乳腺MR平扫及功能扫描，X线及MR检查结果分别与病理结果对比分析。结果 钼靶X线示明显肿块3例，单发或多发结节5例，无明显肿块、不对称的斑片及片状密度增浓影6例，局部结构扭曲及杂乱条索影4例。MR增强扫描均见强化，肿块样强化2例，非肿块样强化6例（结节状、锥状、片结状、多发小结节状及条索状）。动态增强TIC曲线Ⅱ型4例，Ⅰ型2例。ADC值＞1.1×10-3s/mm2者5例。Fisher评分3分1例，2分4例，1分1例。结论 IGM的钼靶X线表现缺乏特征性，结合MR功能成像、Fisher评分，临床综合分析可大大提高诊断准确率。Objective To explore the mammography, magnetic resonance imaging features and clinical characteristics of idiopathic granulomatous mastitis（IGM）. Methods The mammography and MR imaging features and clinical manifestations of 18 cases with surgery and pathology proven IGM were summarized. All the cases were underwent mammographic examinations, MR examination was performed in 6 cases,mammography and MR imaging results were compared with pathological findings, respectively. Results Mammography imaging findings： apparent dense mass was detected in 3 patients, single or multiple nodules in 5 patients, asymmet- ric density was detected in 6 patients and architectural distortion in 4 patients. Enhanced MR imaging：enhancing mass lesion was detected in 2 patients and non-mass-like enhancement focal infiltration in 6 patients. Type II （time-singnal intensity）curve was pictured in 4 cases,type II curve in 2 cases. 5 patients＇ ADC values were more than 1. 1 × 10^-3s/mm^2. The Fisher＇s score ： 3 points in 1 case, 2 points in 4 cases and 1 point in 1 case. Conclusion IGM has no specific mammography findings, combination fMRI and Fisber＇s degree is helpful for clinical diagnosis.
