腹腔镜手术治疗不完全型重复肾畸形(附3例报告)

摘　　要：目的探讨后腹腔镜重复肾切除联合经腹腹腔镜输尿管膀胱再植术治疗不完全型重复肾畸形的手术方法及临床价值。方法2012年2月～2013年8月，采用后腹腔镜重复肾切除联合经腹腹腔镜输尿管膀胱再植术治疗不完全型重复肾畸形3例。术前均经影像学确诊。结果3例手术均获得成功。手术时间141、127、97min。术中出血量82、95、52m1。术后住院时间7、8、7d。随访时间2、8、17个月，彩超及ECT提示正常半肾功能良好，原有腹痛、尿路刺激征、尿失禁等症状消失，未发生严重并发症。结论后腹腔镜重复肾切除联合经腹腹腔镜输尿管膀胱再植术治疗不完全型重复肾畸形具有创伤小、出血量少、术后恢复快、并发症少等优点，可作为一种可行的治疗选择。Objective To investigate the surgical methods and clinical experience of laparoscopic heminephroureterectomy combined with ureterovesical reimplantation for incomplete duplex kidney and ureter anomalies. Methods From February 2012 to August 2013, 3 cases of incomplete duplex kidney underwent retroperitoneal laparoscopic heminephroureterectomy combined with laparoscopic ureterovesical reimplantation. All the cases were confirmed by radiology before the surgery. Results All the operations were completed successfully. The operative time was 141, 127 and 97 min; blood loss was 82, 95 and 52 ml; postoperative hospital stay was 7, 8, and 7 d. All cases were followed up for 2, 8 and 17 months. Color Doppler ultrasound and emission computed tomography showed no intraoperative and postoperative complications. Remaining kidney was unimpaired and primary symptoms and signs disappeared. Conclusions Laparoscopic heminephroureterectomy combined with ureterovesical reimplantation for incomplete duplex kidney and ureteral anomalies is safe and minimally invasive. It has less blood loss, quicker recovery and fewer complications.
