氟康唑联合布替萘芬乳膏治疗念珠菌性包皮龟头炎的疗效观察

摘　　要：目的观察氟康唑联合布替萘芬乳膏治疗念珠菌性包皮龟头炎的临床疗效。方法 80例念珠菌性包皮龟头炎患者随机分为对照组(单用布替萘芬)和治疗组(布替萘芬+氟康唑),每组40例。同时选择40例配偶以布替萘芬+氟康唑治疗,并观察疗效。入组对象以布替萘芬乳膏外涂(1次/d)或布替萘芬乳膏+氟康唑顿服(150 mg/d),连续用药14 d。所有患者分别于治疗前、治疗后第7天及第14天记录临床症状和体征,并做真菌镜检,以判断疗效。结果用药第7天时,对照组和治疗组总有效率分别为65%和70%,真菌清除率分别为60%和80%,差异无统计学意义(P均>0.05)。用药第14天时,对照组和治疗组的总有效率分别为70%和95%(P<0.05),真菌清除率分别为85%和90%(P>0.05)。2组均无明显不良反应发生。配偶组总有效率为100%,真菌清除率为95%。结论氟康唑联合布替萘芬乳膏治疗念珠菌性包皮龟头炎安全有效。Objective To observe clinical effects of flucoanzole combined with butenafine cream in treatment of candidal balano-posthitis. Methods A total of 80 patients with candidal balanoposthitis were randomized into a control group （given butenafine cream only） and a treatment group （given butenafine cream plus fluconazole）, with 40 patients in each group. Forty spouses （spouse group） were selected and given butenafine cream plus fluconazole. Butenafine cream was for external use one time per day, and flu-conazole was used orally 150 mg per day, for 14 days. The clinical signs and symptoms were recorded before treatment, and at the 7th and 14th day of treatment, respectively. Microscopic examination of fungi was performed for determining therapeutic effects. Results At the 7th day of treatment, the total effective rates of the control group and the treatment group were 65%and 70%, fungi clearance rates of the control group and the treatment group were 60% and 80%, respectively, and the differences were not signifi-cant between the two groups （P＆gt;0.05）. At the 14th day of treatment, the total effective rates of the control group and the treatment group were 70% and 95%, and the difference was significant between the two groups （P〈0.05）; fungi clearance rates of the control group and the treatment group were 85%and 90%, respectively, and the difference was not significant between the two groups （P〈0.05）. No obvious adverse reactions were found in the control group and the treatment group . The total effective rate was 100% and fungal clearance rate was 95% in the spouse group. Conclusion Flucozanole combined with butenafine cream is safe and effec-tive in treatment of candidal balanoposthitis.
