摘  要:目的:了解高血压对肾脏血流动力学及功能的影响,分析坎地沙坦的肾脏保护作用。方法:选择高血压组98例,正常组90例,超声多普勒测定收缩期峰值流速(PSV)、舒张末期流速(EDV)、肾动脉阻力指数(RI),生化测定尿素氮(BUN)、肌酐(Cr),肌酐清除率(CCr)和尿微量白蛋白(mAlb),分析两组之间的差异。高血压组治疗12个月后再次测定上述数值,比较治疗前后的改变。结果:高血压组PSV、EDV明显低于正常组,RI、mAlb高于正常组,差异具有统计学意义(P<0.05)。高血压组治疗后,PSV、EDV明显升高,RI、mAlb明显降低,差异具有统计学意义(P<0.05)。结论:肾脏血流动力学检查可以反映高血压对肾脏的早期影响,坎地沙坦具有改善肾血流动力学的作用,并可延缓肾功能不全的进程。Objective To investigate the renal hemodynamic and function in hypertension patients and the renal defencive function of Candesartan.Method 98 hypertension patients and 90 physical examination subjects.The renal hemodynamic were measured by color Doppler ultrasound including PSV,EDV,RI.The renal function were represented by BUN,Cr,CCr and mAlb.The difference between hypertension patients and controls were analyzed.The indexs in hypertension patients were compared after 12 months.Results PSV and EDV in hypertension patients were significanted lower than controls.RI and mAlb were significanted higher than controls.After 12 months,PSV,EDV were increased and RI,mAlb were decreased in hypertension patients.Conclusion Color Doppler ultrasound can provide important information to evaluate abnormal renal function in patients with hypertension.Candesartan may make renal hemodynamic better and protect renal functions.
