摘 要:目的采用基于体素的形态学分析方法(VBM)比较不同部位脑梗死患者脑灰质结构改变。方法对慢性期运动功能恢复良好的25例基底节区梗死、13例脑桥梗死患者和22名性别、年龄相匹配的健康志愿者行高分辨率T1W扫描,采用VBM 8软件包进行图像后处理,分别比较基底节区梗死组、脑桥梗死组与正常对照灰质体积存在差异的脑区。结果基底节区梗死组患侧初级运动皮层(M1区)、丘脑灰质体积减小。脑桥梗死组两侧小脑半球后叶灰质体积减小,以健侧为著;两侧辅助运动区(SMA)、健侧大脑半球M1区灰质体积增加。结论不同部位脑梗死所致脑灰质结构改变存在差异,可能对临床实现个体化治疗及康复训练有一定指导意义。Objective To compare changes of the gray matter structure of patients with different location of cerebral infarction and recovered motor function by using voxel-based morphometry (VBM) method. Methods Twenty-five patients with unilateral infarction of basal ganglia area (basal ganglia group), 13 with unilateral infarction of pons (pons group) and 22 age- and sex-matched healthy participants (control group) were enrolled. High-resolution T1W images were obtained and processed using VBM 8 software package. The gray matter volume of two patients groups were calculated and compared with the control group. Results Compared with control group, the gray matter volume significantly decreased in basal ganglia group including ipsilesional primary motor area (M1) and thalamus. Decreased gray matter volume in bilateral cerebellum posterior lobe, mainly in the contralesional cerebellum was also detected, while the gray matter volume significantly increased in contralesional M1 and bilateral supplementary motor area (SMA) in pons group. Conclusion Changes of gray matter structure caused by different location of cerebral infarction may provide useful references for clinical individual therapy and rehabilitation training.
