摘　　要：目的：调查冠状动脉CTA检查前患者的肾功能及对自己肾功能的了解情况。方法：回顾性分析2369例行CCTA检查的连续患者资料，包括性别、年龄、CT检查前一周内的血清肌酐（Scr）值以及肾功能问卷调查结果，计算得出肾小球滤过率估算值（eGFR）。依据eGFR将患者分成三组：肾功能正常组（eGFR≥90ml／min／1．73m3）、肾功能轻度受损组（90〉eGFR≥60ml／min／1．73m3）和肾功能中重度受损组（eGFR〈60ml／min／1．73m3）。分析各组间年龄、性别、血肌酐及患者对自己肾功能的了解情况。结果：行冠状动脉CTA检查的患者中，32．46％肾功能正常，61．88％肾功能轻度受损，5．66％肾功能中重度受损。三组间性别差异无统计学意义。三组间年龄差异有统计学意义，年龄越大，肾功能受损越严重。肾功能中重度受损的患者中，仅有14．2％患者有Scr水平异常。肾功能正常组，98％的患者对自己肾功能有正确了解。肾功能异常组，只有1．8％～19．6％的患者对自己肾功能有正确了解。结论：CCTA人群以中老年人居多，常合并患者不知情的肾功能不全，CT增强检查前，进行肾功能的筛查是非常有必要的，仅依靠问卷调查或者Scr来判断肾功能是不够的，建议常规用eGFR评估肾功能。Objective： To investigate the renal function and awareness of patients before coronary CT angiography （CCTA）. Methods： A total of 2369 consecutive patients data undergoe coronary CTA were analyzed retrospectively, inclu ding gender,age, serum creatinine （Scr） within one week before examination and renal function questionnaire survey re suits. Estimated glomerular filtration rate （eGFR） was calculated. The patient population was divided into 3 groups （normal group,mild renal impairment group and moderate to severe renal impairment group） according to the eGFR. Age, gender, Scr and kidney function awareness rate were analyzed among the 3 groups. Results： The renal function of 32.46 % patients was normal,61.88% was mildly impaired and 5. 660/oo was moderately to severely impaired. The diffecenee of patient age was statistically significant among the 3 groups. The more elderly patients had lower eGFR. The Scr was abnormal in only 14.2% of the moderate to severe renal impairment group. In patients with normal renal function, 98% of them correctly know their state of kidney. In patients with abnormal renal function, only 1.8 % - 19.6 % of them correctly know their state of kidney. Conclusion： As the middle-aged and the elderly account for the majority examinee, it is necessary to perform kid- ney function screening before CCTA, and eGFR is the recommended screening method, in addition to questionnaire survey and Scr test.
