摘 要:目的i探讨100kVp条件下碘克沙醇(320mgI/mL)应用于冠状动脉CTA(cCTA)检查时,使用不同比例自适应统计迭代重建(ASiR)的图像质量,得出最佳重建参数。方法:前瞻性收集临床疑诊冠脉病变、身体质量指数(BMI)20~25kg/m2的患者资料。所有患者均行64排冠状动脉CTA检查,采用前瞻性心电触发扫描模式,管电压100kVp,管电流600mA。增强扫描使用对比剂碘克沙醇(320mgI/mL),共60mL。采用滤波反投影(FBP)及30%~40%、60%、80%、100%ASiR对原始数据进行重建,得到5组图像。由2名有经验的影像科医师分别对冠状动脉各节段的5组重建图像进行主观评价,包括整体评分和细节评分,并行统计学分析。测量冠脉各节段CT值,定量计算5组图像噪声及对比噪声比(CNR),并行统计学分析。结果:共40例受试者入组,共分析4个血管节段,冠脉各节段图像质量均达到临床诊断要求。30%~40%ASiR组诊断信心明显高于其它各组,且在冠脉边缘锐利度、斑块与冠脉管腔对比度、图像主观噪声方面评分不低于其它各组。共分析16个血管节段,所有冠脉节段平均CT值均〉300HU。不同比例ASiR组的噪声均低于FBP组,cNR均高于FBP组,且各组间差异有统计学意义。结论:100kVP条件下碘克沙醇(320mgI/mL)应用于CCTA检查,可以满足临床诊断需要,推荐使用30%~40%As.R进行图像重建。Objective:To evaluate the image quality of 100kVp CT angiography (CCTA) using iterative reconstruc tion with iodixanol (320rag I/mL). Methods: Patients with clinically suspected coronary artery diseases were prospectively collected, with body mass index (BMI) 20%25kg/mz. Prospective ECCr-triggered CCTA images were acquired with follow ing parameters: 100kVp,600mA;iodixanol 320rag I/mL,total volume 60mL. The raw data were reconstructed with filtered back projection (FBP) and 30 %- 40 %,60 %, 80 %, 100 % adaptive statistical iterative reconstruction (ASiR), respectively. For subjective assessment,2 experienced radiologists assessed both the overall image impression and details of vessel dem- onstration for each segment. For objective assessment, CT value and contrast-to-noise ratio (CNR) of the coronary lumen were measured by segment. Results:Forty cases were enrolled. The image quality of CCTA met clinical diagnostic criteria, both as whole and as segment. Among the CCTA images with different reconstruction algorithm, image with 30 %-40% ASiR was considered to provide higher diagnostic confidence than others. Conclusion: 100kVp CCTA using 30 % -40 % iterative reconstruction with iodixanol (320mg I/mL) is feasible for clinical application.
