多孔钽棒治疗早期股骨头坏死的文献回顾  

The Porous Tantalum Metal Rod for Early Stage of Osteonecrosis of the Femoral Head: A Systematic Review

摘　　要：目的综合分析关于多孔钽棒治疗股骨头坏死的已发表文献,探讨多孔钽棒治疗早期股骨头坏死的疗效及安全性。方法系统检索2000年1月至2012年12月多孔钽棒治疗早期股骨头坏死的相关文献,按照纳入标准和剔除标准进行筛选,提取需要数据,采用循证医学分析方法,加权汇总纳入文献样本的一般资料、临床疗效及并发症指标并进行系统分析。结果共检出112篇相关文献,按照筛选标准筛选后,共有10篇被纳入分析,共有186例患者(223髋),其中男128例、女58例,加权平均年龄为38.3岁,加权平均随访时间12.0个月。加权平均手术时间(34.9±4.9)min,加权平均出血量(57.8±10.0)mL,术后髋关节加权平均Harris评分较术前高[(86.3±3.9)分vs(59.2±11.0)分],差异有统计学意义(t=54.46,P<0.01)。未出现感染、伤口不愈合、深部血管栓塞、骨折、神经损伤、多孔钽棒松动和断裂等其他并发症,仅少数患者出现术后不可解释的疼痛和异位骨化。结论多孔钽棒治疗早期股骨头坏死,手术时间短、出血量少且短期内临床疗效良好。ObjectiveTo systematically review the safety and efficacy of a porous tantalum metal rod for early stage of osteonecrosis of the femoral head.MethodsThe literatures about a porous tantalum metal rod for early stage of osteonecrosis of the femoral head were searched at home and abroad from Jan.2000 to Dec.2012.According to established inclusion and exclusion criteria,literatures met evaluation criteria were selected,and data of general information about sample,clinical efficacy and complications were analyzed by evidence-based medicine methods.Results10 trials involving 186 patients（223 hips） were included in this study.The results showed：weight average age was 38.3 years old,and weight average follow-up was 12.0 months.Weight average operative time was （34.9±4.9） minutes,and weight average operative blood loss was （57.8±10.0） mL.Postoperative weight average Harris score （86.3±3.9） was significantly higher than preoperative score （59.2±11.0）.All patients had no infection,non-healing wounds,or other complications of deep vein thrombosis,fractures,nerve damage,loose and broken porous tantalum metal rod; only a few patients had adverse reactions of unexplainable pain and ectopic ossification.ConclusionThe clinical effect of porous tantalum metal rod for early stage of osteonecrosis of the femoral head is good in short term,with shorter operative time and less blood loss.
