包皮环治疗包皮疾病术后并发症的研究

摘　　要：目的降低一次包皮环切治疗包皮过长和包茎的并发症.方法 对2011年6月至2012年6月在开县人民医院就诊的323例包皮过长和包茎患者随机分为A、B两组,A组160例,B组163例,两组均应用一次性包皮环行包皮环切术.A组术后拆除时间为10 ～14 d,拆除后未行包扎.B组术后拆除时间为术后6 ～7 d,拆除包皮环后采用弹力绷带包扎1周后拆除绷带.结果 A组患者有19例发生水肿、6例吻合口裂开、5例发生包皮感染、4例出现其他并发症;B组患者有7例出现水肿,1例裂开,1例发生包皮感染、2例出现其他并发症.A、B两组并发症发生率分别为21.25%、6.75%,两组比较差异有统计学意义（χ^2＝14.16,P＜0.05）.结论 包皮环治疗包皮过长或者包茎,术后早期拆除包皮环后,采用弹力绷带包扎创缘可显著减少包皮环治疗后的并发症.ObjectiveTo reduce the complications of circumcision procedure for the treatment of phimosis and redundant prepuce.MethodsA total of 323 patients with phimosis from Kaixian County Hospital during Jun.2011 and Jun.2012 were divided into two groups（A and B）randomly：160 cases in group A,163 cases in group B.The two groups were all treated with circumcision by prepuce rings.The rings in group A were removed after 10 to 14 days,and no special handling after that.Group B rings were removed after 6 to 7 days,and the incisions were bandaged with elastic bandage for a week.ResultsThe main complications in group A included penile edema（19 cases）,incision split（6 cases）,infection（5 cases）,the rest（4 cases）; penile edema （7 cases）,incision split（1 case）,infection（1 case）,the rest（2 cases）in group B.The complication rates of the two groups were 21.25%,6.75% respectively.Difference between the two groups was significant（χ2=14.16,P〈0.05）.ConclusionThe complications of prepuce ring treating phimosis and redundant prepuce can be significantly reduced by bandaging the incisions with elastic bandage after early postoperative removing of the prepuce rings.
