尺骨鹰嘴截骨双锁定板固定治疗肱骨髁间粉碎性骨折的疗效观察

摘　　要：目的 观察经尺骨鹰嘴截骨入路双锁定接骨板治疗肱骨髁间粉碎性骨折的疗效.方法 回顾性分析2010年5月至2013年1月涟水县人民医院采用后正中切口尺骨鹰嘴截骨入路双锁定接骨板内固定治疗的24例肱骨髁间C2、C3型骨折患者的临床资料,术后早期行肘关节功能锻炼.根据肘关节改良Cassebaum评分系统评定肘关节的功能.结果 术后24例患者全部获得随访,时间为6 ～18个月,骨折全部愈合.根据Cassebaum评分标准：优13例、良7例、可4例,优良率为83.3%.结论 尺骨鹰嘴截骨为肱骨髁间C2、C3型骨折提供满意术野,双锁定接骨板提供足够坚强的稳定性,允许术后早期功能锻炼,临床疗效满意.Objective To observe the effects of double locking plates on the humeral intercondylar comminuted fractures via the operating approach of elbow and trans-olecranon ostootomy. Methods The clinical data of 24 cases with the humeral intercondylar fractures of type C2 and type C3 from May 2010 to Jan. 2013 in Lianshui People＇s Hospital were reviewed. Posterior midline operating approach of elbow and trans-olecranon osteotomy and internal fixation by double locking plates were performed in all cases. The patients began the active training of elbow joint as soon as possible postoperatively. The elbow function was assessed by Cassebaum modified scoring system. Results All of the 24 eases were followed up for 6-18 months. Bone union was obtained in all cases. According to Cassebaum scoring system：excellent in 13 cases, good in 7, and fair in 4,the excellent and good rate was 83.3%. Conclusion The satisfactory surgery field can be obtained from trans-olecranon osteotomy in the humeral intercondylar fractures of type C2 and type C3 ;double locking plates can provide sufficient stability,which allows early functional exercise after surgery and good clinical effect.
