职业权力监控运行对临床路径开展的效果评价

摘　　要：目的评价实施职业权力监控运行对临床路径开展的效果。方法选取我院内科开展临床路径的部分病种为研究对象,根据职业权力监控运行开展情况,采用对照研究,将监控运行前的2012年患者信息作为对照组,监控运行后的2013年患者信息作为观察组,统计二组临床路径开展情况,评价临床路径执行指标,分析对医疗质量与安全指标的影响。结果观察组住院患者入径率达84.4%、完成率达74.9%,与对照组相比明显增高;患者的平均住院日、次均住院费用显著低于对照组(P<0.05)。入径疾病病死率、医院感染率、出院一周内再住院率、并发症发生率均有不同程度的降低(P<0.05),差异有统计学意义,说明实施监控运行后临床路径积极开展,执行指标成效显著,医疗质量与安全指标进一步提高。结论应重视权力运行的网络信息化建设,加强对职业权力运行的公开监控,积极开展临床路径,规范诊疗行为,不断提高医疗管理水平。Objective To evaluate the effect of clinical pathway of carring out occupation authority to monitor the operation. Methods The part of diseases conducting of clinical pathways in author＇s hos-pital were selected for the study. According to occupational power operation done control study. The control group was the patients＇ information in 2012, before monitoring running. The observer group was the patients＇ information in 2013, after monitoring running. Counted the clinical pathway conduc-ted cases of the two groups, evaluated the clinical pathway implementation indicators and analysis of the impact on health care quality and safety indicators. Results The rate of hospitalized patients into the path was 84.4% and the completion rate was 74.9% in the observer group, which were signifi-cantly higher compared with the control group. The average length of stay of patients and average hos-pitalization costs were significantly lower than the control group（P〈0.05）. The mortality of disease into the path, hospital infection rate, hospital readmission rate within a week and the complication rate had different levels of reduced. The difference were statistically significant （P〈0.05）. Description that the clinical pathway was actively carried out, implementation indicators were remarkable, health care quality and safety indicators were further improve after the implementation of the monitoring op- eration. Conclusions Information network construction of power running should been noticed, strengthen public monitoring occupational power operation, actively carries out clinical pathway, standard treatment behavior to continuously improve the level of medical management.
