摘  要:目的探讨亚甲基四氢叶酸还原酶(MTHFR)C677T基因多态性与老年原发性高血压患者颈动脉粥样硬化斑块形成的关系及影响因素。方法应用彩色超声仪对120例老年原发性高血压患者进行颈动脉测定,其中颈动脉粥样硬化斑块组(斑块组)78例,无斑块组42例;分别用聚合酶链反应检测其MTHFR基因C677T多态性,用酶联免疫吸附法测定血清同型半胱氨酸水平。结果斑块组MTHFR C677T基因TT基因型频率及T等位基因频率与无斑块组比较明显升高(40.4%vs 17.3%,P<0.01;57.0%vs 42.9%,P<0.01)。TT纯合子患者的颈动脉内膜厚度及同型半胱氨酸水平均显著高于CT型和CC型者(P<0.05)。结论 MTHFR C677位点C→T基因突变可能增加了老年高血压患者颈动脉粥样斑块形成的风险。Objective To investigate the effects of Methylenetetrahydrofolate reduetase (MTHFR) gene polymor- phism on the development of carotid atheroselerotic plaque in aged patients with hypertension. Methods The intima - media thickness (IMT) of external carotid common carotid artery and the presence of atheroselerotic plaques were deter- mined by B - mode ultrasonography. A total of 120 aged patients with essential hypertension were assigned to either plaque group (n = 78 ) or non -plaque group (n = 42). Polymerase chain reaction was used for detection of MTHFR gene poly- morphism and enzyme - linked immunoassay ( ELISA ) was applied for measurement of plasma homoeysteine ( Hcy ). Results There was significant difference in the frequencies of genotype and allele in MTHFR C677T gene polymorphism between plaque and control groups ( P 〈 0. 05 ). The IMT and plasma Hcy levels in patients with TT type were significantly higher than patients with CT or CC type (P 〈 0. 05 ). Conclusion MTHFR C677C---T gene mutation may increase the risk of carotid atherosclerotic plaque in aged patients with hypertension.
