剖宫产疤痕妊娠的治疗分析

摘　　要：目的：探讨治疗剖宫产疤痕妊娠安全有效的方法。方法：回顾性分析2008年1月-2013年10月诊治的21例剖宫产疤痕妊娠资料。结果：部分子宫疤痕妊娠者其子宫疤痕处肌层无明显变薄，清宫术出血时缩宫治疗有效；经MTX化疗后B超监测下行清官术可清除妊娠物，但清宫术时机难以控制，并且亦难免出血多；阴式子宫下段疤痕妊娠病灶切除术治疗剖宫产疤痕妊娠操作简便可行，创伤小、出血少、恢复快，手术可同时修复疤痕。结论：对剖宫产疤痕妊娠的治疗应根据患者情况及医疗条件制定最安全有效的治疗方案，阴式子宫下段疤痕妊娠病灶切除术是一种较简单可行、创伤小的方法，并且手术可同时修复疤痕。Objective: To explore a safe and effective method to treat cesarean scar pregnancy( CSP). Methods: The data of 21 patients with CSP treated in the hospital from January 2008 to October 2013 were analyzed retrospectively. Results: For some patients,their myometrium had no significant thinning at scar,oxytocin treatment was effective when bleeding when curettage of uterine cavity. After MTX chemotherapy,curettage of uterine cavity under B- ultrasound guidance could clear away the pregnancy tissues,but the favorable opportunity for curettage uterine cavity was difficult to grasp,and bleeding was unavoidable. Transvaginal surgical removal of scar for CSP was simple and feasible,having the advantages of less wound,less bleeding,and rapid recovery. In addition,it could also repair the scar. Conclusion: A secure and effective treatment program for CSP should be made according to clinical situation and medical conditions. Transvaginal surgical removal of scar is a simple and feasible method with less wound. Furthermore,it can also repair the scar.
