探讨抗CCP抗体在类风湿关节炎诊断中的临床价值

摘　　要：目的探讨抗环瓜氨酸肽抗体(抗CCP抗体)在类风湿关节炎(rheumatoid arthritis,RA)诊断中的临床价值。方法收集2011年1月至12月在解放军四五二医院住院的RA患者150例设为实验组,选择同期在该医院做体检的健康人员150例设为对照组,检测其抗CCP抗体,同时检测类风湿关节炎常用相关诊断指标类风湿因子(RF)和抗角蛋白抗体(抗AKA抗体),并对结果进行统计分析。结果灵敏度、特异性检验,抗CCP抗体、RF和抗AKA的灵敏度分别为78.6%、85.3%和55.3%,特异性分别为96.8%、85.3%和94.7%;kappa一致性分析,以临床诊断作为金标准,抗CCP抗体、RF和抗AKA的kappa系数分别为0.75、0.71和0.50。结论抗CCP抗体对诊断RA具有较高的灵敏度和特异性,是辅助诊断RA的良好指标。Objective To explore the clinical significance of the RA by anti-CCP antibody testing.Methods Choo sing 150 cases of patients with RA for the experimental group and 150 cases healthy check up for the control group,testing the anti-CCP antibody,RF and Anti-AKA antibody,analyze the results comprehensively.Results Compared of the sensitivity and specificity,the sensitivity of the anti-CCP antibody,RF and Anti-AKA antibody is 78.6％,85.3％,55.3％ and the specificity is 96.8％,85.3％,94.7％.Compared of the kappa,the kappa of the anti-CCP antibody,RF and Anti-AKA antibody is 0.75,0.71,0.50.Conclusion The anti-CCP antibody is a good indexes for the RA diagnosis which has high sensitivity and specificity.
