摘 要:目的 调查四川富顺地区不同程度宫颈病变患者人乳头瘤病毒(HPV)感染状况及基因型分布特点,为该地区HPV感染的预防和宫颈癌防治提供理论依据。方法 采用基因芯片技术对宫颈液基细胞技术所获宫颈细胞及手术切除宫颈石蜡组织进行基因分型检查。结果 255例宫颈病变样本中,HPV检出阳性率为63.92%(163/255),其中高危感染占59.22%(151/255),低危感染占20.39%(52/255),单一感染占66.87%(109/163),混合型占33.13%(54/163)。高危型中最常见前6种亚型依次为:HPV16、58、81、52、39和33,低危型中最常见前4种亚型依次为:HPV6、11、43和54。20~29岁与30~39岁年龄段妇女HPV的总感染率明显高于40~49岁年龄段(P<0.05),低危型感染主要集中在20~29岁年龄段,明显高于30~39岁和40~49岁年龄段(P<0.05)。结论 富顺地区不同程度宫颈病变患者有较高的HPV感染率,并且高危型感染率较高,具有较强的区域和年龄段分布特点,为本地区性传播疾病及宫颈癌早期诊断、治疗、预后评估及疫苗使用等提供重要的参考依据。Objective In order to develop a better strategy to prevent and cure cervical cancer in the Fushun area, we investigated the infection subtype of human papilloma virus (HPV) in women with cervical lesions. Methods By using microarray genotyping technology, HPV subtypes were detected in samples from either cervical smear from non-surgical patients or paraffin-embedded cervical tissues from patients with cervical surgery. Results A total of 255 samples were collected in this study. The results indicated that the HPV infection rate was 63.92% (163/255), of which 59.22% (151/255) were infected by the high-risk HPV subtypes, and 20.39% (52/255) were infected by the low-risk HPV subtypes. Of the infected cases, 66.87% (109/163) and 33.13% (54/163) were infected by a single HPV subtype and mixed subtypes, respectively. The most commonly observed high-risk subtypes of HPV were HPV 16, 58, 81, 52, 39 and 33 in order, and the most commonly observed low-risk subtypes of HPV were HPV 6, 11, 43 and 54 in order too. The total HPV infection rate in 20-29 years old and 30-39 years was significantly higher than that in 40-49 years old. For the low-risk HPV subtypes, a significantly higher infection rate was found in people with age from 20 to 29 years old, compared to the other two age groups separately (P〈0.05, respectively). Conclusion The overall HPV infection rate, especial for the high-risk subtypes, in women who live in the Fushun area is high, while different ages of population show different vulnerabilities to different HPV subtypes. This study has provided useful information for our efforts on the development of an area-localized strategy for the prevention, early-diagnosis, the choice of vaccine, and the treatment of this major sexually transmitted disease at the early stage.
