新辅助化疗联合中药治疗头颈部黏膜鳞癌临床观察

摘　　要：目的观察六君子汤合半夏泻心汤加减联合PF方案对头颈部黏膜鳞癌新辅助化疗的减毒增效作用。方法将经病理确诊的50例头颈部黏膜鳞癌患者随机分为治疗组30例和对照组20例。对照组采用PF方案新辅助化疗,治疗组在PF方案新辅助化疗同时加用中药六君子汤合半夏泻心汤加减方,两组均进行2个周期。观察两组肿瘤病灶大小、体质量、生活质量、毒副反应、免疫功能、KPS评分、中医症状积分等指标变化。结果两组均完成2个周期新辅助化疗,治疗后治疗组及对照组白细胞计数、血红蛋白浓度、血小板计数、肿瘤大小、KPS评分与治疗前比较,差异有统计学意义(P<0.05),而中医症状积分仅治疗组治疗前后差异有统计学意义(P<0.05)。治疗后两组中医症状积分差异有统计学意义(P<0.05),而肿瘤病灶缩小情况两组间差异无统计学意义(P>0.05)。结论 PF方案联合六君子汤合半夏泻心汤加减能减轻头颈部黏膜鳞癌新辅助化疗毒副反应,提高患者耐受能力,辅助化疗顺利完成。Objective To investigate the efficacy-increasing and toxicity-decreasing effects of herbal prescription of the six noble herbs plus the revised pinellia on PF regiment for adjuvant chemotherapy of head and neck mu-cosal squamous cell carcinoma （MSCC）. Methods Fifty pathological diagnostic patients with head and neck MSCC were randomly divided to experimental group（n=30） that was given 2 cycles of PF regiment chemotherapy plus herbal medicine and control group （n=20） that was only given 2 cycles of PF regiment chemotherapy. The efficacy was evaluated after treatment from tumor size, weight, quality of life, side effects, immune function, KPS score, and score of traditional Chinese medicine syndromes. Results All patients received 2 cycle chemotherapy. Significant difference was noted in WBC, Hb, PLT, tumor size, and KPS between before and after chemotherapy in both groups（P〈0.05）, and was found in the score of traditional Chinese medicine syndrome only in the experimental group（P〈0.05）. The score of traditional Chinese medicine syndrome showed significant difference between experi-mental and control groups after treatment（P〈0.05）. Clinical efficacy did not differ between two groups（P＆gt;0.05）. Conclusion The herbal prescription of the six noble herbs plus the revised pinellia is of help to reduce side ef-fects of chemotherapy in patients with head and neck MSCC treated with PF regiment, improve the patients＆#39; toler-ance and ensure the accomplishment of chemotherapy.
