常规皮瓣移植与封闭负压引流技术联合皮瓣修复骶尾部压疮对比研究

摘　　要：目的探讨封闭负压引流技术(VSD)联合皮瓣治疗骶尾部压疮的适应证、方法及临床疗效。方法回顾性分析2006年11月—2013年11月,应用VSD联合皮瓣修复136例骶尾部压疮(VSD组)临床资料,并与同期单纯皮瓣修复的100例(非VSD组)进行比较。结果两组修复术前住院时间、总住院时间以及应用皮瓣面积差异有统计学意义(P<0.05);修复术后住院时间差异均无统计学意义(P>0.05)。皮瓣修复术后VSD组2例、非VSD组12例皮瓣远端出现皮缘部分坏死,清创植皮或直接缝合后创面愈合良好;其余皮瓣全部成活,创面Ⅰ期愈合。结论 VSD联合皮瓣修复对及时控制骶尾部皮肤软组织缺损感染、改善创面血运、缩短术前准备时间、及早闭合创面、促进愈合有显著作用。Objective To explore the indications, methods and curative effect of vacuum sealing drainage （VSD） combined with flaps in repair of sacral pressure sores. Methods Clinical data of 136 patients with sacral pres- sure sores repaired by VSD combined with flaps （VSD group） during November 2006 and November 2013 was retrospec- tively analyzed, and was compared with that of 100 patients repaired by only flaps （non-VSD group）. Results The differences of preoperative hospitalization days, total hospitalization days and flap areas in the two groups were statistically significant （ P 〈 0.05 ） ; the difference of postoperative hospitalization days was not statistically significant （ P 〉 0.05 ）. All flaps survived except for 14 cases of partial necrosis （2 cases in VSD group, 12 cases in non-VSD group） , and the 14 flaps healed well after debridement and skin grafting or suturing, and the wound surfaces were stage I healing. Conclusion VSD combined with flaps in repair of sacral pressure sores may control infection of skin soft tissue loss quickly, improve blood supply, shorten preoperative hospitalization days, close wound surfaces and help healing effectively.
