摘　　要：目的探讨个案管理对糖尿病患者治疗效果的影响。方法对92例糖尿病患者实施个案管理,分析管理前后患者的血糖及相关代谢疾病情况。结果个案管理1年后,患者血糖得到有效控制,患者治疗依从性明显提高,体重及相关并发症得到良好控制,管理前后比较差异明显(P<0.01或P<0.05)。结论个案管理能有效提高糖尿病患者的自我管理能力,控制血糖及并发症的发生。Objective To explore the effects of the treatment of diabetes in case management. Methods For 92 patients with diabetes case management, patients before and after the analysis of the management of blood sugar and related metabolic diseases. Resnlts Case management 1 years later, the patient to effectively control blood sugar, obviously improve the patients＇ treatment compliance, weight and related complications get good control, management comparison before and after significant difference （P〈0.01 or P〈0.05）. Conclusion The ease management can effectively improve patient self-management ability, so as to control blood glucose and complications.
