动态血糖监测下超短效与短效胰岛素经胰岛素泵皮下输注治疗2型糖尿病临床观察

摘　　要：目的:比较超短效与短效胰岛素经胰岛素泵皮下输注治疗2型糖尿病的临床疗效。方法:选取我院2009年2月至2012年10月收治的70例2型糖尿病患者按照随机数字表法随机分为A组与B组各35例,分别给予经胰岛素泵皮下输注超短效胰岛素、短效胰岛素治疗,治疗期间均采用动态血糖监测系统进行血糖水平监测,治疗4周后比较两组患者各项指标变化。结果:两组患者FPG、2hPG及睡前血糖水平治疗前、治疗后组间比较无明显差异(P>0.05),但A组达标时间及胰岛素用量显著少于B组(P<0.01);两组患者治疗前各项指标比较无明显差异(P>0.05),治疗后A组较治疗前、同期B组血糖均值、血糖标准差、血糖≤3.9mmoL/L时间百分比均明显低于B组(P<0.05),具有统计学意义。结论:超短效胰岛素经胰岛素泵皮下输注治疗2型糖尿病具有疗效佳、安全性高等优点,值得推广应用。Objective：To compare the clinical efficacy of ultra short-term effect insulin and short-term effect insulin in the treatment of type 2 diabetes by insulin pump subcutaneous infusion .Method：70 cases of type 2 diabetes patients in our hospital from Feb .2009 to Oct.2012 were selected ,they were randomly divid-ed into group A and group B , with 35 cases in each group , They were treated with ultra short-term effect in-sulin and short-term effect insulin by insulin pump subcutaneous infusion .They were all used continuous glucose monitoring system to monitor blood glucose levels , all indexes were compared between the two groups after 4 weeks treatment .Result：FPG, 2hPG and bedtime blood glucose levels of the two groups had no sig-nificant difference from that before treatment and after treatment （P＆gt;0.05）, but the standard time and the dosage of insulin in group A were significantly less than that in group B （ P＆lt;0.01）;All the indexes of the two groups of patients had no significant difference from that before treatment （ P＆gt;0.05）.After treatment mean blood glucose , blood glucose , blood sugar percentage standard deviation of time was less than 3.9mmoL/L in group A were significantly lower than before treatment and the same period group B （P＆lt;0.05）, with statisti-cal significance .Conclusion：The ultra short-term effect insulin by subcutaneous infusion of insulin pump has good curative effect , high security in the treatment of type 2 diabetes , it is worthy of popularization and application .
