摘 要:目的:通过检测基质金属蛋白酶-9(MMP-9)在子痫前期患者胎盘中的表达情况,探讨其与子痫前期发病的关联及与胎盘重量、新生儿体重的关系。方法:采用免疫组化SP法检测60例子痫前期(研究组)、30例妊娠妇女(对照组)胎盘组织中MMP-9的表达水平,结果以光密度值半定量表示,其中轻度(MPE组)、重度(SPE组)子痫前期患者各30例。结果:(1)MMP-9在胎盘中的光密度值研究组低于对照组,SPE组低于MPE组(P<0.05)。(2)研究组胎盘重量、新生儿体重低于对照组,SPE组低于MPE组(P<0.05)。(3)各组胎盘的MMP-9表达水平与胎盘重量、新生儿体重呈正相关(P<0.05)。(4)MMP-9在胎盘中的平均光密度值新生儿体重<2 500 g者低于≥2 500 g者(P<0.05)。结论:子痫前期患者胎盘MMP-9呈低表达,随着病情加重,其表达减弱,提示其可能参与子痫前期的发病,并与胎儿生长受限相关。Objective To discuss the relationship between matrix metalloproteinases 9 (MMP-9) and the pathogenesis of preeclampsia, birth weight and placental weight, through detecting the expression of MMPS-9 in the placenta of the patients with preeclampsia. Methods The levels of MMP-9 in the placenta were detected by immunohistochemical SP method in 60 patients with preeclampsia and 30 pregnancy women. Results (1) The optical density value of MMP-9 in preeclampsia group was lower than the control group, the optical density value of MMP-9 in SPE group was lower than the MPE group (P 〈 0.05). (2) The placenta weight and birth weight in preeclampsia group were lower than the control group, the placenta weight and birth weight in SPE group was lower than the MPE group (P 〈 0.05). (3) The expression level of MMP-9 were positively correlated with placental weight and birth weight (P 〈 0.05) in the control group. (4) The average optical density value of MMP-9 in neonatus with weight 〈 2 500 g was lower than neonatus with weight 1〉2 500 g (P 〈 0.05). Conclusion The MMP-9 in the placenta of patients with preeclampsia was down-regulated, and its expression was abate when the illness grew worse. The MMP-9 may be involved in the pathogenesis of preeclampsia, and it is associated with fetal growth restriction.
