摘 要:目的观察六味地黄汤对慢性肾功能衰竭(CRF)大鼠肾小管上皮细胞-间充质转化(EMT)的影响。方法将60只SD大鼠随机分正常组10只,假手术组10只,模型组20只,六味地黄汤组20只,模型组和六味地黄汤组进行5/6肾切除术建立CRF大鼠模型,假手术组分离肾被膜不切除肾脏,正常组不作任何处理。六味地黄汤组予以六味地黄汤灌胃[生药6.75g/(kg·d)],其余各组以等体积蒸馏水灌胃,每天1次,共给药8周。用免疫组织化学、Western Blot、RT-PCR方法检测各组大鼠肾组织转化生长因子-β1(TGF-β1)和骨形成蛋白-7(BMP-7)及其mRNA表达的差异。结果与假手术组比较,模型组TGF-β1和BMP-7蛋白表达面密度值增加(P<0.01),平均灰度值降低(P<0.01),TGF-β1 mRNA表达水平上调(P<0.01),BMP-7 mRNA表达水平下调(P<0.01);与模型组比较,六味地黄汤组TGF-β1表达面密度值降低,平均灰度值增加(P<0.01),BMP-7表达面密度值增加,平均灰度值降低(P<0.01),TGF-β1mRNA表达水平下调,BMP-7 mRNA表达水平上调(P<0.01)。结论六味地黄汤通过下调TGF-β1蛋白及其mRNA表达水平,上调BMP-7蛋白及其mRNA表达水平,抑制CRF大鼠肾小管的EMT,可能是抗肾间质纤维化的作用机制。Objective To observe the effects of Liuwei Dihuang Decoction ( Six-Ingredient Rehmannia Decoc- tion, 六味地黄汤) on the tubular epithelial-mesenchymal transformation (EMT) in rats with chronic renal failure (CRF). Methods Sixty SD rats were randomized into the normal group (n = 10), sham operation group (n = 10), model group (n =20) and Liuwei Dihuang Decoction group (n =20). The CRF models were established with 5/6 nephrectomy in the model group and Liuwei Dihuang Decoction group. The renal capsule was isolated in the sham operation group without removal of the kidney. There was no treatment in the normal group. The Liuwei Dihuang Decoction group was gavaged with Liuwei Dihuang Decoction [ 6. 75 g/ ( kg ·d) raw drug ]. The other groups were gavaged with an equal volume of distilled water once daily for 8 weeks. The expressions of transforming growth factor- β1 (TGF-β1), TGF-β1 mRNA, bone morphogenetic protein-7 (BMP-7) and BMP-7 mRNA were detected with im- munohistochemistry, WesternBlot and RT-PCR. Results Comparing with the sham operation group, the expres- sions of TGF-β1 and TGF-β1 mRNA were significantly increased but the expressions of BMP-7 and BMP-7 mRNA were significantly decreased in the model group ( P 〈 0. 01 ). Comparing with the model group, the expressions of TGF-I31 and TGF-[31 mRNA were significantly decreased but the expressions of BMP-7 and BMP-7 mRNA were sig- nificantly increased in the Liuwei Dihuang Decoction group ( P 〈 0. 01 ). Conclusion Liuwei Dihuang Decoction (六味地黄汤) can inhibit tubular EMT in rats with CRF possibly by decreasing the expressions of TGF-β1 and TGF- β1 mRNA and increasing the expressions of BMP-7 and BMP-7 mRNA, which may be its mechanism of anti-renal interstitial fibrosis.
