摘  要:目的:观察电针足三里、天枢穴对肠易激综合征模型大鼠肠系膜微血管管径及血浆中降钙基因相关肽(CGRP)、血管活性肠肽(VIP)水平的影响,探讨针刺治疗IBS的可能机制。方法:以Wistar幼鼠为研究对象,分为正常组、模型组、天枢组、足三里组,天枢和足三里组,采用联合作用型方法制备IBS大鼠模型,从第6周束缚及电针干预,隔日1次,共7次。模型组只束缚不针刺,正常组不做任何处理。治疗结束后,采用XW-B-3冷光源微循环显微仪观察各组大鼠肠系膜微循环不同时间段的管径;采用酶联免疫法(Elisa)检测大鼠血浆中CGRP、VIP的含量。结果:①肠系膜微血管管径:与正常组比较,模型组血管管径在即刻、10分钟、30分钟时明显收缩(P<0.05);与模型组比较,天枢组、足三里组血管管径扩张,天枢组20分钟P<0.05,即刻、30分钟P<0.01,足三里组各时间段血管管径与模型组比较(P<0.01)。②血浆CGRP、VIP含量:与正常组比较模型组大鼠血浆中VIP含量升高(P<0.05);电针足三里组大鼠血浆中CGRP、VIP含量升高(P<0.05、P<0.01)。与模型组比较电针足三里组大鼠血浆中CGRP升高(P<0.01)。结论:肠易激综合征模型大鼠表现为明显的微循环功能障碍,血浆中血管内皮相关物质CGRP、VIP水平紊乱。针灸可有效调节紊乱的CGRP、VIP水平,进而扩张微血管,改善微循环功能障碍,起到治疗作用。Abstract Objective: To explore the mechanism of acupuncture in treating irritable bowel system(IBS) by ob- serving acusector at ZuSanLi (ST36) and TianShu (ST25) on microvascular caliber, the levels of CGKP and VIP in IBS rat model. Methods: The rats were divided to normal group, model group, TianShu group, ZuSanLi group, Tian- Shu and ZuSanLi group, the models were established by the methods, bounded from the 6th week and intervened with acusector, once every other day, for seven times. The model group were bounded and the normal group left un- handled. When the treatment was finished, the caliber ofmesenteric capillaries was observed in different times with XW-B-3 cold light microcireulation tester; the contents of CGRP and VIP in the plasma were detected by ELISA method. Results: ① Microvascular caliber of mesentery: compared with the normal group, vascular caliber of the model group narrowed obviously immediately, in ten minutes and 30 minutes (P〈0.05); compared with the model group, vascular caliber of TianShu group and ZuSanLi group expanded, TianShu group in 20 minutes P〈0.05, in- stantly, in 30 minutes P〈 0.01, ZuSanLi group at different times P〈 0.01. ② The contents of CGRP and VIP in the plasma: compared with the normal group, the contents of VIP increased in the model group (P〈0.05); the contents of CGRP and VIP in the plasma increased in ZuSanLi group (P〈0.05, P〈0.01). Compared with the model group, the contents of CGRP raised in the plasma of ZuSanLi group (P〈0.01). Conclusion: IBS rat model shows obviousmicrocirculation dysfunction and the disorder of regulating CGRP and VIP in the plasma. Acupuncture could dilate the capillary and improve microcirculation dysfimction by regulating the levels of CGRP and VIP.
