我国HIV／TB双重感染者血清细胞因子水平的研究

摘　　要：目的比较分析我国广西地区单纯结核(TB)患者、人类免疫缺陷病毒(HIV)感染者及HIV/TB双重感染者(双感者)血清细胞因子水平。方法选择2010年4月至2010年9月间纳入广西地区未接受高效抗逆转录(HAART)治疗HIV/TB双感者15例、接受HAART治疗HIV/TB双感者5例、未治疗单纯HIV感染者13例、HAART治疗单纯HIV感染者4例、单纯TB患者10例及健康对照17例。使用Bio Plex悬浮芯片系统检测血清IL.2、IL.4、IL.6、IL.8、IL.10、IFN.γ、TNF.α、GM.CSF等8种细胞因子。结果与健康对照相比,单纯HIV感染者IL.8水平显著降低,单纯TB患者IL.10及TNF.α水平升高,HIV/TB双感者IL.10及TNF.α升高更加显著,结核痰涂片镜检阴性及阳性双感者间未见显著性差异。HIV/TB双感者与单纯HIV感染者相比,前炎性因子IL.6、IL.8、IFN.γ、TNF.α等显著升高。与未治疗者相比,接受治疗的HIV/TB双感者血清TNF.α水平出现下降趋势。结论TB与HIV单独及相互作用使单纯TB患者、HIV感染者及V/TB双感者体内细胞因子失衡,加速了疾病进展。Objective To compare and analyze the serum cytokine level among HIV, TB and HIV/TB co-infection patients. Methods Fifteen HAART naive and five HAART HW/TB co-infection patients, thirteen HAART naive and four HAART HIV infection patients, ten TB patients and seventeen healthy control people were included in this study between April 2010 and September 2010. The levels of patients＇ IL-2,IL-4,IL-6,IL-8, IL-10,IFN-γ,TNF-α and GM-CSF in serum were detected by Bio Plex system. Results Compare to the healthy control,IL-8 is higher in HIV infec- tion patients and IL-10,TNF-α are higher in TB patients and highest in the HIV/TB co-infection ones. There is no difference in cytokines between the sputum smear positive and negative HIV/TB co-infection patients. HIV/TB co-infection patients have higher IL-6, IL-8, IFN-γ, TNF-α than the HIV ones. There is a recovery tendency in the level of TNF-α among the HIV/TB co-infection patients with HAART compared to those HAART negative ones. Conclusion It is the interaction between the HW and TB that cause the cytokine imbalance in HIV/TB co-infection patients.
