假性膜部瘤与真性膜部瘤室间隔缺损介入封堵术心律失常发生率的比较与分析

摘　　要：目的分析假性膜部瘤与真性膜部瘤室间隔缺损介入封堵术心律失常的发生率,并探讨其防治措施。方法将227例成功进行介入封堵治疗室间隔缺损患者分为假性膜部瘤组(n=160)与真性膜部瘤组(n=67),每例患者术前均经临床体检、心电图、胸部X线、超声心动图检查确诊,对其围术期持续心电监测数据进行比较与分析。结果真性膜部瘤组术后各种心律失常发生率较假性膜部瘤组明显增加(P<0.01),其中以传导阻滞发生尤为明显。结论真性膜部瘤室间隔缺损介入封堵术后心律失常发生率显著增加,与其解剖结构有关。因此,术中操作轻柔,选择合适的封堵器,术后严密观察,积极治疗,绝大多数患者均可恢复。Objective To analyze the incidence of arrhythmia in pseudo theca tumor and true theca tumor by tran- scatheter closure of ventricular septal defect treatment,and explore its prevention measures. Methods 227 patients who were successful transcatheter closure treatment ventrieular septal defect,they were divided into pseudo theca tumor group （n：160） and true theca tumor group （n=67）. Each patient preoperatively diagnosed by clinical examnatlon,elec- trocardiogram,chest X-ray,ultrasound diagnosed beckoning diagram.And the data of perioperative continuous ECG monitoring was compared and analyzed. Results The incidence of variety arrhythmia postoperative in true theca tumor group increased significantly compared with the pseudo theca tumor group （P〈0.01）,among them,the conduction block was most obvious. Conclusion The incidence of arrhythmia in pseudo theca tumor and true theca tumor by tran- scatheter closure of ventricular septal defec is significant increase,it is related to its anatomical structure.As a result,in- traoperative soft operation,select the appropriate plugging device,postoperative close observation and active therapy,most patients can recover.
