摘　　要：目的探讨阿托伐他汀对高血压合并动脉粥样硬化患者内皮功能的影响。方法选取在本院就诊的高血压合并颈部动脉粥样硬化患者80例作为实验组,另选取80例无颈动脉斑块的高血压患者作为对照组。实验组在标准高血压治疗方法的基础上加用阿托伐他汀治疗,对照组仅应用高血压治疗。检测并比较颈动脉内膜中层厚度(IMT)、粥样硬化斑块积分、血管内皮依赖性舒张功能(FMD)、非内皮依赖性舒张功能(NMD)、测定血超敏C反应蛋白(hs-CRP)、血脂水平和空腹血糖(FBG)。结果治疗前实验组与对照组相比,IMT和hs-CRP较高,而FMD值较低,差异有统计学意义(P<0.05)。在接受阿托伐他汀治疗之后,实验组患者FMD显著改善,IMT、硬化斑块积分以及hs-CRP显著减低(P<0.05)。结论高血压合并动脉硬化患者动脉内膜厚度高于正常值并且存在血管内皮功能障碍,阿托伐他汀能有效改善血管内皮功能,在一定程度上逆转动脉粥样硬化病症。Objective To explore influence of atorvastatin on endothelial function in patients with atherosclerosis and hypertension. Methods 80 patients with hypertension and carotid artery atherosclerosis were collected and designed as experimental group, while 80 hypertension patients without carotid plaque were designed as control group. The control group was treated with the stan- dard hypertension treatment, while the experimental group was treated with atorvastatin treatment on the basis of the control group. Indexes such as carotid intima- media thickness （IMT）, atherosclerotic plaque score, vascular endothelium-dependent vasodilatation （FMD）, endothelium- dependent vasodilatation （NMD）, serum high-sensitivity C-reactive protein （hs-CRP）, lipid levels and fasting blood glucose （FBG） were detected and compared between two groups. Results Before treatment, IMT and hs-CRP levels in the experimental group were significantly higher than those in the control group, while FMD value was significantly lower （ P 〈 0.05）. After atorvastatin thera- py, FMD in the experimental group improved significantly, and IMT, hardened plaque score and hs -CRP reduced significantly （P 〈 0.05）. Conclusion Artery intima thickness in the patients with hypertension and arteriosclerosis is higher than the normal level, and most of them have endothelial dysfunction. To a certain degree, atorvastatin can improve endothelial function and reverse atherosclerosis disease.
