氯胺酮减轻心肺转流后中性粒细胞激活作用的临床观察

摘　　要：目的：探讨全身麻醉诱导时应用小剂量氯胺酮对冠状动脉搭桥术后中性粒细胞产生超氧化物的影响。方法30例择期冠状动脉搭桥手术患者分为两组，在芬太尼诱导的基础上分别加用小剂量氯胺酮（氯胺酮组）或等量生理盐水（对照组），血样采集时间点分别为：手术前、体外循环后即刻、术后1～6 d。中性粒细胞功能采用（12-）十四酸佛波酯（-13-）乙酸盐、酵母多糖或甲酰-甲硫-亮-苯丙氨酸刺激后的超氧化物产量测定法。结果全身麻醉时复合小剂量氯胺酮可抑制超氧阴离子的增加，此外氯胺酮可降低术后2～6d的中性粒细胞百分比。结论氯胺酮可减轻心肺转流后中性粒细胞的激活。Objective To investigate the application of small dose of ketamine during induction of anesthesia in patients after coronary artery bypass grafting neutrophil superoxide generation effect .Methods 30 patients undergoing elective coronary artery bypass grafting operation were randomly divided into 2 groups ,in the fentanyl induced respectively based on combined with small dose of ketamine （ketamine group） or normal saline （control group） ,a blood sample collection time points ：before ,immediately after extracorporeal circulation operation ,operation after 1-6 days .Neutrophil function by using （12-） fourteen acid and phorbol ester （-13-） acetate （PMA） ,yeast polysaccharide or formyl-methylthio-light-phenylalanine after stimulation of superoxide production de-termination method .Results During general anesthesia combined with low dose ketamine inhibits superoxide anion increases .In ad-dition ,ketamine reduces perioperative 2 -6 days of the neutrophil percentage .Conclusion Ketamine can reduce the activation of neutrophils after cardiopulmonary bypass .
