摘 要:目的 探讨超声对训练伤致跟腱断裂的诊断价值.方法 93例部队训练致小腿及足踝部损伤患者行跟腱超声检查,其中21例经MRI及手术确诊为跟腱断裂(病例组),另外72例无跟腱断裂(对照组),观察跟腱断裂的超声声像图改变及特点,并用诊断试验评价其诊断价值.结果 跟腱断裂的超声表现为跟腱纤维结构连续性完全或部分中断,断裂处有不均质回声或无回声区.超声诊断闭合性跟腱断裂的灵敏度71.4%,特异度88.9%,真实性84.9%,阳性预测值65.2%,阴性预测值91.4%.结论 超声能显示跟腱断裂的部位及程度,准确性较高,具有一定的临床实用价值.Objective To explore diagnostic value of ultrasonography in achilles tendon rupture caused by military training. Methods Ultrasonography was performed in 93 patients with shank, ankle and foot injury caused by military training. 21 cases were diagnosed as achilles tendon rupture by MRI and operation( case group) ,and 72 cases with no rupture( control group). Changes and characteristics of ultrasonic image were observed and evaluated by diag- nostic test. Results Ultrasonography showed that fiber structure continuity of tendon was completely or partially inter- rupted. Heterogeneity echo area or anechoic area was found within rupture. The sensitivity, specificity, accuracy, positive and negative predictive values were 71.4% ,88.9% ,84.9% ,65.2% and 91.4% respectively. Conclusion Ultrasonography can show the location and degree of achilles tendon rupture with high accuracy, which has a certain clinical value.
