上海市徐汇区全科医师开展基层版《中国高血压防治指南》培训的效果评价

摘　　要：目的 了解社区全科医师开展高血压健康宣教情况及培训需求、基层版〈中国高血压防治指南〉相关知识掌握情况.方法 采用便利抽样方法选取上海市徐汇区5家社区卫生服务中心的147名全科医师,以所在中心为单位分为干预组（78人）和对照组（69人）,对干预组实施为期8个月围绕〈指南〉的专题培训,于培训前后分别进行问卷调查.结果 147名全科医师中有92.5%（136/147）曾经接受过相关知识培训,并希望继续接受培训者占87.8%（129/147）.本次培训前后干预组的高血压知识问卷得分有明显提高（P=0.025）,而对照组无变化（P=0.078）;特别是高血压治疗方面的知识,培训前干预组的答题正确率明显低于对照组（50.1%比59.3%,P=0.001）,通过系列培训后干预组的正确率较对照组有显著提高（65.5%比 55.4%,P=0.000）.结论 通过高血压知识培训结果可以看到,采用定期讲座的形式对社区医师进行培训具有一定效果.三级医院应发挥自身优势,对基层医师定期开展针对〈指南〉的培训和指导,以提高其对高血压诊疗知识的掌握.Objective To evaluate the effect of ＂The Guidelines for Prevention and Treatment of Hypertension in China＂ （the Guidelines） training in Shanghai General practitioners （GPs）. Methods One hundred and forty seven GPs from 5 community health centers in Xuhui District of Shanghai were assigned into intervention group（ n = 78） and control group （n = 69）. GPs in intervention group received regular trainings of the Guideline for 8 months, the control group did not rceeive the training. A questionnaire survey was eonducted in both groups hefore and after the training. Results Most of GPs in the study （92. 5% ） had received hypertension training courses before, and majority of them （87.7%） wished to receive the further training. The scores of knowledge about hypertension in intervention group increased significantly （P = 0. 025 ） after training, but there were no changes in the control group （P = 0. 078 ） before and after training. The correct rate of hypertension treatment in intervention group was lower than that of control group before training （50. 1% vs. 59. 3% , P = 0. 001 ） ; however, the correct rate in intervention group was significantly higher than that of control group （65.5% vs. 55.4% , P = 0. 000） after training. Conclusions Providing GPs with regular training course on the Guidelines would improve their clinical ability for treatment of hypertension in the community.
