脑心通胶囊联合阿托伐他汀钙对老年颈动脉粥样硬化斑块的影响

摘　　要：目的观察脑心通胶囊联合阿托伐他汀钙治疗老年颈动脉粥样硬化斑块的临床疗效。方法经超声检查确诊为颈动脉粥样硬化伴斑块形成的老年患者306例,随机分为观察组与对照组,观察组(156例)给予阿托伐他汀钙20 mg,每晚1次,脑心通胶囊3粒(1.2 g),3次/d,疗程为6个月;对照组(150例)采用阿托伐他汀钙20 mg,每晚1次。于治疗前,治疗2、6个月时,检测颈动脉彩超、肝肾功能、血脂、凝血功能、血液流变、糖化血红蛋白、hs-CRP等指标的变化,以观察临床效果。结果观察组治疗6个月后,颈动脉内膜-中层厚度(IMT),斑块横切面最大面积(Smax),斑块数量、低密度脂蛋白(LDL)、高敏C反应蛋白(hs-CRP)等指标均较对照组改善明显,组间比较差异有统计学意义(P<0.05),且2组患者治疗前后肝肾功能无明显改变。结论脑心通胶囊联合阿托伐他汀钙治疗老年颈动脉斑块,能通过减轻炎症反应,保护血管内皮细胞等机制达到治疗动脉硬化的目的。Objective To study the clinical effect of Naoxintong capsule combined with atorvastatin calcium on elderly patients with carotid atherosclerosis plaque. Methods 306 cases of older patients diagnosed with carotid atherosclerosis plaque were randomly divided into the observation group and the control group. The observation group was given atorvastatin calcium （20 mg, one time a night） combined with Buchang NaoXinTong capsule （3 capsules （1.2 g）, 3 times a day, 6 months as one course of treatment）. The control group was given atorvastatin calcium （20 mg, one time a night）. The carotid artery with color Doppler ultrasound, liver and kidney function lipid, blood coagulation function, glycosylated hemorrheology, hsCRP and other indicators were detected before and after the treatment for 2,6 months. The clinical effect of Naoxintong capsule com- bined with atorvastatin calcium on elderly patients with carotid atherosclerosis plaque was observed. Results The intima- middle thickness （IMT）, largest area of plaque transverse section （Smax）, plaque quantity, low density lipoprotein （LDL） and hsCRP indexes were more improved in observation group than those of control group after treatment for 6 months. There was statistically significant difference between groups（ P 〈 0.05）. The function of liver and kidney has no obvious change in patients of two groups before and after treatment. Conclusion Naoxintong capsule combined with atorvastatin calcium on el- derly patients with carotid atherosclerosis plaque is safe and effective to significantly reduce the incidence of cerebrovascular diseases.
