摘  要:目的对无锡市人群、动物及节肢类虫媒生物新布尼亚病毒感染情况,分析感染谱、流行病学特点及影响因素,为控制疫情提供依据。方法采用统一的诊断标准调查病例,双抗原夹心酶联免疫吸附测定法(enzymelinked immunosorbent assay,ELISA)法检测血清特异性总抗体,采用逆转录-聚合酶链反应(reverse transcription-polymerase chain reaction,RT-PCR)法进行病原学诊断。结果 2010-2011年,共调查发热伴血小板减少综合征(severe fever with thrombocytopenia syndrome,SFTS)疑似病例66例,疑似病例确诊阳性率为7.58%,确诊病例病死率为60.00%(3/5)。一般人群总抗体阳性率为0.23%(2/866);家畜类动物337只,抗体阳性率为2.08%,阳性率较高的动物为鹅(33.33%)、羊(6.76%)和鸡(0.67%);野生类动物115只,抗体阳性率为1.74%,阳性率较高的动物为鼠(2.13%)。节肢类虫媒生物中未发现核酸检测阳性者。结论初步确认新布尼亚病毒感染谱包括隐性感染和显性感染(包括轻症病例、重症病例和死亡病例);该病可能为人畜共患病,可能宿主动物为鼠、羊和鸡;年龄偏大和生活在丘陵地区可能是SFTS发病的危险因素。Objective To investigate and analyze the epidemic status, infection spectrum, epidemiological characters and risk factors of the new bunia virus in human, animals and arthropods in Wuxi City. Methods Cases were investigated with the unified diagnostic criteria. Overall antibodies to severe fever with thrombocytopenia syndrome were detected by a novel double antigen sandwich ELISA. Reverse transcription-PCR(RT-PCR) was also used for nucleotide diagnosis. Results A total of 66 suspected SFFSV-specific antibodies were found in Wuxi City, and the positive rate of suspected SFTSV-specific antibodies was 7.58%. The mortality rate was 60.00% (3/5). The positive rate of the total antibody was 0. 23 % (2/866). SFTSV-specific antibodies were found in total number of 337 domestic animals (2. 08% ) and 115 wild animals ( 1.74% ). The higher positive rates of animals were geese (33.33%), sheep (6. 76% ) and chicken (0. 67% ) in domestic animals, and mice (2. 13% ). Nucleic acid detections were all negative with arthropods. Conclusions The results indicated that the spectrums of infection of SFTSV include the latent infection and the apparent infection ( with slight, serious and dead cases). The disease might be perhaps a zoonosis. Mice, sheep and chicken can be host animals. The elderly and those who live in hilly areas can be risk factors of the SFTS.
