辨证分型治疗慢性胃炎合并溃疡性结肠炎随机平行对照研究

摘　　要：[目的]观察辩证分型治疗慢性胃炎合并溃疡性结肠炎疗效。[方法]使用随机平行对照方法，将86例门诊及住院患者按抽签方法分为两组。对照组43例柳氮磺砒啶开始时4g／次，1次／d；21～28d后2g／次。1次／d．口服，治疗维持1～2年；加氢化可的松，200～300mg／次，1次／d，口服；或结合地塞米松，10mg／次，1次／d。开始时大剂量，7～14d后，改用泼尼松，60mg/次．1次／d．口服。待病情缓解以后逐渐减量直到停药。治疗组43例辨证分型①脾胃虚弱型健脾益气、调和肠胃、温化湿寒．四神汤、香砂六君子汤。②寒热错杂型，健脾养阴、寒热并治，驻车丸，半夏泻心汤。③肝郁气滞型，健脾和胃、疏肝理气．痛泻要方。④胃肠湿热型，调和脾胃、化湿清热，香连丸。两组均连续治疗30d为1疗程。观测，临床症状、不良反应。治疗1疗程，判定疗效。[结果]治疗组治愈16例．显效14例，有效10例，无效3例，总有效率93．02％。对照组治愈14例，显效13例．有效9例．无效7例．总有效率83．72％。治疗组疗效优于对照组（P〈0．05）。[结论]辨证分型治疗慢性胃炎合并溃疡性结肠炎效果显著．值得推广。[ Objective ] Observe dialectical type treatment of chronic gastritis combined with ulcerative colitis . [ Methods ] Using randomized controlled method, 86 cases of outpatient and inpatient divided into two groups by drawing of lots . Control group, 43 cases of sulfasalazine pyridine beginning 4g / time, 1 time / d; 21 28d after 2g / times, 1 times / d, orally, treatment for 1 to 2 years ; hydrogenated cortisone, 200 - 300mg / times, 1 times / d, orally ; or in combination with dexamethasone , 10mg / times, 1 times / d. The beginning of large doses , 7 - 14d after the switch to prednisone , 60mg / times, 1 times / d, orally. To be in remission after tapering until withdrawal. Treatment group 43 cases dialectical type treatment , based on clinical symptoms differences and tongue and pulse were divided into the following four types . （1） weak stomach type Spleen Qi , to reconcile the stomach, cold temperature and dampness , Sishen soup , Xiangsha Liujunzi . （2） mixed cold and heat type , spleen yin, chilis and fever and cure , parking pills, BanxiaxiexinTang. （3） liver Qi stagnation, spleen and stomach, liver qi, Tongxieyaofang . （4） Gastritis , to reconcile the stomach, dampness heat , Hong pill . 30d continuous treatment in both groups as a course of treatment. Observation of clinical symptoms and adverse reactions. Treatment of a course of treatment, to determine efficacy . [ Results ] treatment group, 16 patients were cured , 14 cases markedly effective in 10 cases, 3 cases, the total efficiency of 93.02% . Control group, 14 cases were cured , 13 cases were markedly effective in 9 cases, 7 cases, the total efficiency of 83.72% . Treatment group than the control group （ P〈0.05 ） . [ Conclusion ] dialectical type ulcerative colitis treatment of chronic gastritis combined effect is remarkable , worthy of promotion.
