慢性心力衰竭老年患者中医辨证分型与甲状腺激素和肌钙蛋白I、尿酸水平相关性临床观察

摘　　要：目的:观察老年慢性心力衰竭(CHF)中医辨证分型与甲状腺激素、肌钙蛋白I、血尿酸水平的关系。方法:将93例CHF患者分为6个证型组,检测各组甲状腺激素、肌钙蛋白I、血尿酸水平,并与对照组(22例)相同指标相比较。结果:TT3、FT3水平CHF心肾阳虚证组、气虚血瘀证组、阳虚水泛证组、痰饮阻肺证组均明显降低;肌钙蛋白I水平心肾阳虚证组、气虚血瘀证组、阳虚水泛证组、痰饮阻肺证组均明显升高。尿酸水平阳虚水泛证组较对照组明显升高,痰饮阻肺证组较对照组、心肺气虚证组、气阴两亏证组、心肾阳虚证组明显升高。结论:甲状腺激素、肌钙蛋白I、血尿酸水平可以作为老年CHF中医辨证分型的依据。Objective：To observe the relativity between chronic heart failure （CHF） differentiation types of TCM and thyroid gland hormone, cTn - I and serum uric acid level. Methods ： CHF patients were divided into 6 groups, measuring their thyroid gland hormone,cTn -I and serum uric acid level and comparing them to that of control group（22 cases）. Results ：TI3 and FT3 levels were both much more reduced in CHF groups of yang deficiency of heart and kidney, blood staSis due to qi deficiency and yang deficiency and water accumulation and phlegm and fluid retention obstructing lung. Cardiac Troponin- I levels were raised obvi- ously in CHF groups of yang deficiency of heart and kidney, blood stasis due to qi deficiency and yang deficiency and water accu- mulation and phlegm and fluid retention obstructing lung. In CHF yang deficiency and water accumulation group, serum uric acid level was much more raised than that of control group. In phlegm and fluid retention obstructing lung group, serum uric acid level was much more raised than that of control group and groups of qi deficiency of heart and lung, simultaneous deficiency of qi and yin and yang deficiency of heart and kidney. Conclusion ： Thyroid gland hormone, eTn - I and serum uric acid level may be the ba- sis for TCM differentiation types of senile patients of CHF.
