针刺用于桡骨远端骨折手法复位的镇痛作用研究

摘　　要：【目的】探讨针刺用于桡骨远端骨折复位患者的镇痛作用研究。【方法】将60例桡骨远端骨折患者随机分为治疗组、对照组、空白组3组,每组20例。3组均按诊疗常规施行桡骨远端骨折手法复位、小夹板固定术;治疗组接诊时给予针刺相应穴位镇痛治疗,对照组接诊时给予盐酸曲马多口服治疗,空白组接诊时不予任何止痛处理。分别在接诊时、针刺/服药后15 min、复位后即时、复位后15 min采用McGill疼痛量表进行疼痛评分,并于复位后即时采用模拟评分法(VAS)进行疼痛评分。【结果】(1)在针刺/服药后15 min、复位后即时、复位后15 min等时间点,治疗组McGill量表的疼痛总分均低于对照组和空白组,差异有统计学意义(P<0.05)。(2)治疗组复位后即时VAS疼痛评分显著低于对照组和空白组,差异有统计学意义(P<0.05)。【结论】针刺可用于桡骨远端骨折手法复位的镇痛,在急性疼痛期(<1 h)较盐酸曲马多的镇痛效果更佳。Objective To discuss the acupuncture analgesia effect on distal radial fractures （DRF） treated by manipulative reduction. Methods Sixty DRF patients were evenly randomized into 3 groups, namely acupuncture group, western medicine group and blank control group. Patients in all of the three groups were gi＇gen con, cer^tional manipulative reduction and small splintage fixation, and additionally, acupuncture group was given acupuncture for relieving pain, western medicine group was treated with Tramadol Hydrochloride Tablets orally for relieving pain, and blank control group had no medication. Pain scores were evaluated by McGill pain questionnaire at admission time, 15 minutes after acupuncture or medication, immediately after manipulative reduction and 15 minutes after manipulative reduction. Results The total pain scores evaluated by McGill pain questionnaire in acupuncture group were lower than those in western medicine group and blank control group 15 minutes after acupuncture or medication, immediately after manipulative reduction and 15 minutes after manipulative reduction （P〈0.05）, and pain scores evaluated by visual analog scale （VAS） in acupuncture group were also lower immediately after manipulative reduction （P 〈0.05）. Conclusion Acupuncture shows better analgesia effect than Tramadol Hydrochloride Tablets at acute pain period （less than one hour after fractures） during the manipulative reduction for DRF.
