摘 要:【目的】探讨益气化瘀补肾方股密葆对大鼠激素性股骨头坏死骨代谢的影响。【方法】选用2月龄清洁级SD大鼠180只,随机分为空白对照组,模型组,福善美组(剂量为40 mg/kg),股密葆方高、中、低剂量组(中药高、中、低剂量组,剂量分别为24.3、12.15、6.075 g·kg-1·d-1),每组8只,雌雄各半。采用大肠杆菌内毒素联合激素注射的造模方法复制大鼠激素性股骨头坏死模型。造模成功后,各治疗组分别按设计剂量灌胃给药,于药物干预治疗的第4周末每组取6只大鼠进行骨代谢相关数据的观察和比较。【结果】中药高剂量组可显著降低I型胶原C端肽(CTX-Ⅰ)水平(P<0.05);中药中剂量组可显著升高1,25羟维生素D3[1,25-(OH)-D3]、I型前胶原羧基端前肽(PICP)水平,降低CTX-Ⅰ水平(P<0.05或P<0.01);中药低剂量组可显著升高1,25-(OH)-D3水平,降低CTX-Ⅰ水平(P<0.01或P<0.001),其中降低CTX-Ⅰ水平作用优于福善美组(P<0.05)。【结论】益气化瘀补肾方股密葆具有促进骨形成,抑制骨吸收,增加骨密度的作用,可延缓或阻止激素性股骨头坏死的发生。Objective To observe the effects of Gumibao, a Chinese herbal recipe with the actions of replenishing qi, activating blood and invigorating kidney, on bone metabolism of steroid-induced osteonecrosis of the femoral head (SOFH) in rats. Methods One hundred and eighty 2-month-old clean SD rats were randomly divided into 6 groups, namely blank control group, model group, Fosamax group (40 mg/kg), and high-, middle- and low-dose Gumibao groups (at the dosages of 24.3,12.15,6.075 g'kg-1·d-1, respectively), 8 rats in each group, male in half. SOFH model was established by injection of endotoxin plus steroid according to S. Okazaki's modeling method. After successful establishment of the rat model, the medication groups were given the corresponding medicine orally according to the experimental design, and blank control group and model group were given equal volume of normal saline orally. At the end of the forth week of medication, 6 rats in each group were used for the determination and comparison of bone metabolism data. Results In comparison with the model group, C-terminal telopeptide of type 1 collagen (CTX - I ) level was significantly decreased in high-dose Gumibao group (P〈0.05), and 1,25-dihydroxy vitamin D3 and procollagen type 1 C-terminal peptide (PICP) were increased while CTX- I was decreased in middle-dose Gumibao group (P〈O.05 or P〈0.O1 ) . Low-dose Gumibao had an obvious effect on increasing 1,25-dihydroxy vitamin D3 and on decreasing CTX- I expression level (P〈O.O1 or P〈0.001), and the effect on decreasing CTX- I was superior to that of Fosamax group (P〈 0.05). Conclusion Gumibao, which has the actions of replenishing qi, activating blood and invigorating kidney, exerts certain effect on promoting bone formation, inhibiting bone resorption, increasing bone mineral density, thus to delay and prevent the occurrence of SOFH.
