超声对胆囊息肉样病变M值测定的临床价值

摘　　要：目的：通过测定超声下胆囊息肉样变的M值协助临床诊断及指导制定治疗方案。方法：选取我院自2009年4月-2013年4月收治的胆囊息肉样病患者280例,所有患者均行B超检查,观察并记录超声下息肉样病变部位、数目、大小、形态及胆囊壁厚度等指标,运用胆囊息肉M值进行测定并对所得数据及术后病理类型进行统计学分析。结果：所有患者非肿瘤性息肉250例,肿瘤性息肉30例,M值〈0者212例均为良性病变,0〈M值〈2.5者43例,恶性病变11例,良性病变32例；M值〉2.5者25例,恶性病变19例,良性病变6例, PLG病理类型发现其发病率与M值评分呈正相关关系,M值越高其发病率越高（r=1.224, P〈0.01）。结论：超声下M值测定对PLG的诊断及治疗具有指导意义,有助于尽早识别胆囊癌及恶变可能性的息肉,具有广阔临床应用价值。Objective：Through the determination of ultrasound of gallbladder polypoid change M value for clinical diagnosis and guiding therapy.Methods：280 cases in our hospital from 2009 April to 2013 April were gallbladder polypoid disease patients,all patients underwent B-mode ultrasonography examination,observe and record the ultrasound polypoid lesions,the number,size,shape and thickness of gallbladder wall of gallbladder polyps,using M were measured and the statistical analysis of pathological types were obtained data and postoperative.Results：All patients with non-neoplastic polyps in 250 cases,30cases of tumor polyp,M〈0 in 212 cases were benign lesions,0〈M〈2.5 cases,11cases of malignant lesions,32cases of benign lesions;M value〉25 cases 2.5,19 cases of malignant lesions,6 cases of benign lesions,pathologic types of PLG show that the incidence rate of correlation between the scores were positively and M,M value is higher the higher incidence （r=1.224,P〈0.01）.Conclusion：Determination of the diagnosis and treatment of PLG is meaningful conclusions ultrasound M value,contribute to the early identification of gallbladder carcinoma and malignant potential of the polyp,and has wide clinical application value.
