布地奈德混悬液雾化治疗氨中毒所致呼吸道损害效果观察

摘  要:目的:探讨布地奈德混悬液雾化治疗氨中毒所致呼吸道损害临床效果。方法:选择41例急性氨中毒患者,随机分为两组,对照组20例,实施吸氧、维持水电解质酸碱平衡以及抗生素预防和治疗感染等措施,观察组21例,在对照组的基础上实施布地奈德混悬液氧气雾化吸入,比较两组患者治疗后3天肺功能以及血气分析结果,并统计中毒相关并发症发生情况。结果:观察组FEV1、FEV1/FVC、MVV高于对照组(P<0.05),血氧饱和度、动脉血氧分压和pH值高于对照组(P<0.05),动脉血二氧化碳分压低于对照组(P<0.05),治疗期间发生呼吸衰竭、急性肺水肿、喉头水肿及气道黏膜坏死比例低于对照组(P<0.05)。结论:早期使用布地奈德氧气雾化能较快地缓解患者缺氧和二氧化碳蓄积,减轻患者肺功能损伤,还能减少并发症,是一种安全有效的治疗方法。Objective:To explore the effect of budesonide oxygen atomization on respiratory tract damage induced by ammonia poisoning.Methods:41 cases of acute ammonia poisoning patients were randomly divided into two groups,the control group(20 cases) were used the implementation of oxygen,water and electrolyte acid-base balance and antibiotics maintenance measures,the observation(21 cases)were used budesonide oxygen atomization based in the control group,then the lung function and blood gas the analysis results,and complications related to statistical poisoning in the two groups were compared after 3 days.Results:In the observation group,FEV1,FEV1/FVC,MVV were higher than control group(P〈0.05),oxygen saturation,arterial partial pressure of oxygen and pH value were higher than control group(P〈0.05),arterial partial pressure of carbon dioxide was lower than control group(P〈0.05),respiratory failure,acute pulmonary edema,laryngeal edema and airway mucosal necrosis ratio were lower than control group during the treatment period(P〈0.05). Conclusion:Budesonide oxygen atomization can quickly relieve early hypoxia and carbon dioxide retention with patients,and it also can reduce the lung function injury and the complications of patients,so it is a safe and effective treatment method.
