小儿烧伤血浆内毒素水平变化的临床研究

摘　　要：目的：探讨小儿烧伤后血浆内毒素水平变化的规律,为临床治疗提供参考。方法：随机选择2011年11月-2013年2月本院烧伤科132例烧伤患儿,按烧伤程度分为轻度烧伤组、中度烧伤组和重度烧伤三组,同时选取体检中心的43例正常健康患儿作为对照组。入院时检测血浆内毒素水平,分别和对照组相比。然后动态检测三组患儿血浆内毒素水平,探索变化规律。结果：入院时检测血浆内毒素,轻度和中度与对照组相比差异无统计学意义（P〉0.05）,重度与对照组相比差异有统计学意义（P〈0.05）。动态检测中,入院8h时,中、重度组和轻度组相比,差异有统计学意义（P〈0.05）,重度和中度相比,差异也有统计学意义（P〈0.05）。随时间推移内毒素水平上升,第四天达到高峰,14天左右轻、中度恢复正常,重度稍高于正常值。结论：小儿烧伤后血浆内毒素水平明显升高,其升高程度与烧伤程度成正比。Objective：To discuss the changes of plasma endotoxin level after burn pattern ,providing the reference for the clincal treatment. Methods：Randomly selected 132 cases of pediatric burn in burn unit in hospital from November 2011 to February 2013. Divided into mild burn group, moderate group and severe burn group according to the degree of burn, and selected 43 healthy children in our physical examination center as control group. Measured the plasma endotoxin level on admission,compared with the control group respectively. Then gave the plasma endotoxin level of the three groups children dynamic test, to study the regularity of changes. Results：Plasma endotoxin detection on admission, no statistical significance in mild and moderate compared with the control group （P〉0.05）, compared severe group with the control group ,there was significant difference （P〈0.05）. Dynamic detection, admission 8h, compared moderate group and severe group with mild group, the difference was statistically significant （P〈0.05）, compared severe with moderate , there was statistically significant differences （P〈0.05）. With the passage of time endotoxin level rise, reaching a peak at fourth days, 14 days or so light, moderate and severe back to normal, slightly higher than the normal value. Conclusions：The levels of endotoxin in plasma were significantly elevated after burn, and proportional to the degree of burn.
