Reference Intervals of Serum Calcium,Phosphate and Alkaline Phosphatase of Healthy School Children and Adolescents in Inner Mongolia

摘  要:目的调查分析内蒙古自治区健康青少年血清总钙、磷和碱性磷酸酶水平,建立不同性别和不同年龄健康青少年血清总钙、磷和碱性磷酸酶的参考范围。方法采用分层、随机整群抽样的大样本临床流行病学调查,于2009年7月~2010年6月,从内蒙古自治区随机抽取2324名9~18岁健康中小学生进行调查,筛除离群数据个体后,共2319名受试者入选,男1070名,女1249名,测定血清总钙、磷和碱性磷酸酶浓度。建立不同性别、不同年龄健康青少年血清总钙、磷和碱性磷酸酶的参考范围(P2.5~P97.5)。结果描述了9~18岁青少年男性和女性的血清总钙、磷和碱性磷酸酶的均值、标准差、第2.5和97.5百分位数。男性血清总钙浓度(2.51±0.09,2.49~2.53mmol/L)与年龄无相关性(r=-0.035,P=0.256),女性血清总钙浓度(2.49±0.10,2.47~2.54mmol/L)与年龄呈负相关(r=-0.129,P〈0.001),每增加1岁,血清总钙浓度降低O.005mmol/L。男性血清磷浓度(1.63±0.24,l.31~1.79mmol/I。)和碱性磷酸酶浓度(246±82,130~292U/L)从9岁开始逐渐升高(r分别为0.137和0.153,P〈0.001),每增加1岁,血清磷和碱性磷酸酶浓度分别增加0.023mmol/L和9U/L,至12岁最高,然后逐渐下降(r分别为-0.647和-0.661,P〈0.001),每增加1岁,血清磷和碱性磷酸酶分别降低0.09mmol/L和38U/L;女性血清磷浓度(1.52±0.24,1.31~1.76mmol/L)和碱性磷酸酶浓度(180±95,82~288U/L)在10岁年龄段最高,然后随年龄增加逐渐下降(r分别为-0.682和-0.786,P〈0.001),每增加1岁,血清磷和碱性磷酸酶分别降低0.063mmol/L和31U/L。青少年人群的血清总钙、磷和碱性磷酸酶水平显著高于该研究同期的内蒙古地区成年人(男性t分别为10.64,52.90和61.56;女性t分别为16.15,Objective To analyze the serum total calcium,phosphate and alkaline phosphatase level among apparently healthy school children and adolescents in Inner Mongolia and establish the reference intervals of serum total calcium, phosphate and alkaline phosphatase in different gender and different age groups. Methods Large sample clinical epidemiological investiga tion was applied by two stage clustering sampling method. Random sample of 2 324 primary and secondary school students from 9 to 18-year-old was selected from Inner Mongolia from July 2009 to June 2010. Serum total calcium, phosphate and al- kaline phosphatase concentration were measured. After screening outlier individual,a total of 2 319 subjects were enrolled in- volving 1 070 male and 1 249 female. Percentile was used to describe the distribution of serum total calcium, phosphate and alkaline phosphatase level of different growth stages and the reference intervals of serum total calcium, phosphate and alka line phosphatase for healthy school children and adolescents were established by gender and age (P2.5 to P97.5). Results The mean, standard deviation, 2.5th and 97.5th centile values of serum total calcium, phosphate and alkaline phosphatase for boys and girls were presented. No correlation was seen in serum total calcium values (2.51 ±0.09,2.49 ~ 2.53 mmol/L) with age in boys (r=-0. 035,P:0. 256). But in girls serum total calcium (2.49±0.10,2.47~2.54 mmol/L) showed a negative correlation with age (r=- 0. 129, P〈0. 001) with each year increase in age, serum total calcium decreased by0. 005 mmol/L. In boys the highest serum phosphate and alkaline phosphatase level was seen at 12 years. Serum phosphate (1.63±0.24,1.31~1.79 retool/L) and alkaline phosphatase (246±82,130-292 U/L) showed a positive correlation with age (r=0. 137 and 0. 153 ,respectively,P〈0. 001) with each year increase in age,serum phosphate and alkaline phosphatase increased by 0. 023 mmol/L and 9 U/L, respectively.
